Short answer: The Xyosted label starts at 75 mg injected under the skin of the abdomen once a week. A trough testosterone level, drawn 7 days after the last dose, is checked after 6 weeks, 6 weeks after any change, and periodically. At 650 ng/dL or more the dose drops 25 mg; under 350 ng/dL it rises 25 mg. Strengths are 50, 75 and 100 mg.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Xyosted is approved for: Xyosted is approved as testosterone replacement therapy in adult males with low or absent natural testosterone due to primary hypogonadism (testicular failure) or hypogonadotropic hypogonadism (a pituitary or hypothalamus cause). The label says safety and efficacy have not been established in men with "age-related hypogonadism" or in males under 18 (section 1).

Xyosted is injected subcutaneously (under the skin) in the abdominal region only, once a week, using a single-dose autoinjector. The label says to avoid injecting into a muscle or a blood vessel (sections 2.2, 2.3).
The liquid is checked before use and not used if it is cloudy or has visible particles, or if the seal is broken. Any unused portion is thrown away (section 2.3).
It comes as 0.5 mL autoinjectors in three strengths: 50 mg, 75 mg and 100 mg (section 3).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Total testosterone trough (Ctrough) | What the label says to do |
|---|---|
| ≥650 ng/dL | Decrease the dose by 25 mg |
| ≥350 ng/dL and <650 ng/dL | Maintain the same dose |
| <350 ng/dL | Increase the dose by 25 mg |
Before starting, hypogonadism is confirmed by morning serum testosterone measured on at least two separate days, with both results below the normal range.
The starting dose is 75 mg once a week, injected under the skin of the abdomen.
The trough is total testosterone measured 7 days after the most recent dose. It is checked after 6 weeks of dosing, 6 weeks after any dose change, and periodically during treatment.
A trough between 350 ng/dL and 650 ng/dL generally gives testosterone levels in the normal range for the whole week between doses.
In the clinical studies, everyone started at 75 mg weekly and was titrated to 50 mg or 100 mg weekly as needed to reach a pre-dose level of at least 350 and under 650 ng/dL.
Hematocrit is checked before starting and about every 3 months. If it is high, Xyosted is stopped until it falls; if it rises again after restarting, Xyosted is stopped permanently.
Blood pressure is checked periodically, especially in men with high blood pressure. Xyosted is not recommended with uncontrolled high blood pressure.
The lipid profile is checked periodically, particularly after starting testosterone.
The Xyosted label gives no missed-dose instruction. Ask your prescriber what to do if a weekly injection is late.
More on this: missed a dose of Xyosted.
In Dose Track you set up Xyosted as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
