Dose Track reminds you on your Zepbound day and logs each shot, whether it comes from a single-dose pen, a vial or a KwikPen, and keeps count of what is left. Your weight sits next to every dose step from 2.5 mg up.
Per the FDA label (2.1, 2.2), Zepbound starts at 2.5 mg once weekly for 4 weeks, then 5 mg. The dose may rise in 2.5 mg steps after at least 4 weeks on the current dose. Weight-management maintenance is 5, 10 or 15 mg weekly; OSA maintenance is 10 or 15 mg; the maximum is 15 mg.
| When | Dose on the label | Label note |
|---|---|---|
| Weeks 1–4 | 2.5 mg once weekly | Initiation dose; the label says 2.5 mg is not approved as a maintenance dosage |
| Weeks 5–8 | 5 mg once weekly | Lowest weight-management maintenance dose |
| Weeks 9–12 | 7.5 mg once weekly | Escalation step (+2.5 mg after at least 4 weeks); not listed as a maintenance dose |
| Weeks 13–16 | 10 mg once weekly | Maintenance dose option |
| Weeks 17–20 | 12.5 mg once weekly | Escalation step; not listed as a maintenance dose |
| Week 21 onward | 15 mg once weekly | Maximum dosage for all indications |
Zepbound steps up by 2.5 mg after at least 4 weeks on each dose. In Dose Track each step is recorded from its first day. The label says 2.5 mg is for starting only, not a maintenance dose, so the step history is worth keeping.
Zepbound comes as single-dose pens, single-dose vials (one dose each, drawn up with a syringe) and a 4-dose KwikPen. If you use vials, the app logs the dose in mg; each vial is one shot.

Each row is something worth writing down for Zepbound, why it matters for this medicine, and what Dose Track does with it.
| What to track | Why it matters | In Dose Track |
|---|---|---|
| Shot day | The label gives Zepbound once weekly, any time of day, with or without meals (2.4). | A weekly reminder on your day, and a prompt if you log on a different one. |
| Dose step | Start at 2.5 mg for 4 weeks, then 5 mg; 2.5 mg steps after at least 4 weeks, up to 15 mg (2.1, 2.2). | Record each new dose from the day it starts; the log keeps every step. |
| Pens or vials left | Single-dose pens and vials come in cartons; the KwikPen holds 4 doses (3, 16.1). | Dose Track has the Zepbound pen and KwikPen sizes from the label; each shot takes one dose off, with run-out and order-by dates. |
| Weight | Zepbound is labeled for weight reduction with diet and activity, and for obstructive sleep apnea in adults with obesity (1). | Log weight or sync from Apple Health or Health Connect; it charts next to your doses. |
| Injection site | Abdomen or thigh, or the back of the upper arm when another person injects, rotated each dose (2.4). | Each shot is logged with its site and the next one is suggested. |
| Side effects | Nausea, diarrhea, vomiting and constipation were the most common reactions in the label's trials (6.1). | Quick picks for GLP-1 side effects, with severity and notes. |

Per the label, the majority of nausea, vomiting and/or diarrhea events occurred during dose escalation and decreased over time; gastrointestinal adverse reactions overall occurred in 56% at each Zepbound dose versus 30% on placebo. Most patients who stopped Zepbound for adverse reactions did so during the first few months, due to gastrointestinal reactions (6.1).
Dose Track has quick picks for the side effects people on GLP-1 medicines log most: nausea, constipation, diarrhea, reflux or heartburn, tiredness, low appetite, injection-site reaction and headache. You can type in anything else, set how bad it was, and see it next to the dose and the dose step you were on.
Zepbound side effects from the FDA label
Per the label, a missed dose is given as soon as possible within 4 days (96 hours); if more than 4 days have passed, the missed dose is skipped and the next dose is given on the regular day. The weekly day can be changed if the two doses are at least 3 days (72 hours) apart. (2.3) Missed a Zepbound dose.
In Dose Track a missed dose stands out in your history, so you can see the gap and count from the right day.
Dose Track is a tracking app, not medical advice. It records what you and your prescriber decide; it does not recommend doses. Check any dose change with your prescriber.
