Missed a Foundayo Dose? What the FDA Label Says
[Missed dose · Foundayo]

I missed my Foundayo dose. What should I do?

orforglipron tablets · One tablet once daily, with or without food · Label checked

Short answer: The Foundayo label says to take a missed tablet as soon as possible and not to double up the next dose. Never take 2 tablets in the same day. If you miss 7 or more doses in a row, the label says to restart dose escalation at a lower dose, so call your prescriber before restarting.

What does the Foundayo label say about a missed tablet?

Section 2.3 of the Foundayo prescribing information says that if a dose is missed, take it as soon as possible, and do not double up the next dose. The Medication Guide adds: do not take 2 doses of Foundayo in the same day, and do not take more than 1 tablet per day.

Example: you usually take Foundayo at 8 a.m. and realize at 3 p.m. that you forgot. Under the label you would take it at 3 p.m. and take the next one the following day. If you only notice the next morning, you take that day's single tablet, not two.

What if I missed a week or more of Foundayo?

This is the rule that matters most. Section 2.3 says that if 7 or more doses in a row are missed, dose escalation should be restarted at a lower dose to reduce the risk of stomach and gut side effects. The Medication Guide tells patients who miss 7 or more days in a row to call their healthcare provider about how to restart.

Foundayo starts at 0.8 mg once daily and steps up after at least 30 days on each dose (section 2.1), so the restart dose is a decision for your prescriber.

Why is one missed Foundayo tablet less of a problem than a missed week?

Orforglipron has an elimination half-life of about 29 to 49 hours (section 12.3), so the level falls over a couple of days, not hours. One missed day leaves some drug on board. After a full week off, most of it is gone, which is why the label resets the dose steps.

How does Foundayo compare with Rybelsus?

Both are daily GLP-1 tablets, but the rules differ. The Rybelsus label says to skip a missed tablet and take the next one the following day, and Rybelsus has to be taken on an empty stomach. The Foundayo label says to take the missed tablet as soon as possible, and it can be taken with or without food.
