Short answer: The Tlando label gives one dose with no titration: 225 mg, taken as two 112.5 mg capsules, by mouth twice daily, once in the morning and once in the evening, with food. Serum testosterone is checked 8 to 9 hours after the morning dose, 3 to 4 weeks after starting. Tlando continues if the level is 300 to 1080 ng/dL and is stopped if outside that range.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Tlando is approved for: Tlando is approved as testosterone replacement in adult males with low or absent natural testosterone due to primary hypogonadism (testicular failure) or hypogonadotropic hypogonadism (a pituitary or hypothalamus cause). The label says safety and effectiveness have not been established in males under 18 or in men with age-related ("late-onset") hypogonadism (section 1).

Tlando is taken by mouth twice a day: two capsules in the morning and two capsules in the evening, with food (section 2.3; Medication Guide).
Each capsule is 112.5 mg, so each dose is 225 mg (sections 2.3 and 3).
Before starting, low testosterone is confirmed by morning blood tests on at least two separate days, both below the normal range (section 2.2).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Serum testosterone (8 to 9 hours after the morning dose) | What the label says |
|---|---|
| 300 - 1080 ng/dL | Continue TLANDO |
| < 300 ng/dL | Discontinue TLANDO |
| > 1080 ng/dL | Discontinue TLANDO |
The recommended dosage is 225 mg (two 112.5 mg capsules) twice daily, morning and evening, with food. The label does not describe raising or lowering the dose.
Serum testosterone is measured 8 to 9 hours after the morning dose, 3 to 4 weeks after starting and periodically after that, and the result decides whether Tlando is continued or stopped.
Tlando is not substitutable with other oral testosterone undecanoate products.
Hematocrit is checked before starting, about every 3 months in the first year and every 6 months after that.
Serum prolactin is checked before starting and again 3 to 4 months after starting; Tlando is stopped if it stays high.
Prostate cancer evaluation, including PSA, is done before starting and during treatment.
Blood pressure is monitored periodically, especially in men with high blood pressure, and the lipid profile is checked periodically (section 5.12).
The Tlando label gives no missed-dose instruction. Ask your prescriber or pharmacist what to do if a dose is missed.
The label says Tlando is not substitutable with other oral testosterone undecanoate products. It gives no conversion from other testosterone forms.
In Dose Track you set up Tlando as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
