Short answer: The label says to start Wegovy injection at 0.25 mg once weekly for weeks 1 through 4, then 0.5 mg for weeks 5 through 8, 1 mg for weeks 9 through 12 and 1.7 mg for weeks 13 through 16. From week 17 the maintenance dose depends on the use, usually 2.4 mg or 1.7 mg. For adult weight loss the maximum is 7.2 mg once weekly.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Wegovy is approved for: The label says Wegovy injection is approved, with a reduced-calorie diet and more physical activity, to lower the risk of major heart events in adults with heart disease and obesity or overweight, to reduce excess weight and keep it off in adults and children 12 and older with obesity (and adults with overweight plus a weight-related condition), and to treat MASH with moderate to advanced liver fibrosis in adults. Wegovy tablets are approved for the heart-risk and weight uses in adults.

Wegovy injection is given under the skin of the abdomen, thigh or upper arm once a week, on the same day each week, at any time of day, with or without meals. Injection sites are rotated with each dose.
Wegovy tablets are taken once daily on an empty stomach in the morning with up to 4 ounces of water only, swallowed whole, with a wait of at least 30 minutes before food, drinks or other oral medicines.
Both forms are used together with a reduced-calorie diet and increased physical activity. In people with diabetes, the label says to check blood sugar before starting and during treatment.

| Phase | Days | Once Daily Tablet Dosage |
|---|---|---|
| Starting Dosage | 1 through 30 | 1.5 mg |
| Dosage Escalation | 31 through 60 | 4 mg |
| Dosage Escalation | 61 through 90 | 9 mg |
| Maintenance Dosage | 91 and onward | 25 mg |
Heart risk reduction in adults: the maintenance dose is 2.4 mg (recommended) or 1.7 mg once weekly, chosen by response and tolerability.
Weight reduction in adults: the maintenance dose is 1.7 mg or 2.4 mg (recommended) once weekly. If 2.4 mg is tolerated for at least 4 weeks and more weight loss is clinically indicated, the dose may be raised to a maximum of 7.2 mg once weekly.
Weight reduction in children 12 and older: the maintenance dose is 2.4 mg (recommended) or 1.7 mg once weekly.
MASH with moderate to advanced liver fibrosis in adults: the maintenance dose is 2.4 mg once weekly; if not tolerated it can be lowered to 1.7 mg, with re-escalation to 2.4 mg considered.
If a dose is not tolerated during escalation, the label says delaying the next step by 4 weeks may be considered.
Tablets: the maintenance dose is 25 mg once daily. If 25 mg is not tolerated, the label says switching to Wegovy injection 1.7 mg once weekly may be considered.
The label does not recommend using Wegovy with other semaglutide products or any other GLP-1 receptor agonist.
The label says to stop Wegovy at least 2 months before a planned pregnancy because of the long half-life of semaglutide.
Injection: if one dose is missed and the next scheduled dose is more than 2 days away, give the missed dose as soon as possible; if it is less than 2 days away, skip it and resume on the regular day. If 2 or more doses in a row are missed, restart dose escalation at a lower dose. Tablets: skip the missed dose and take the next dose the following day.
More on this: missed a dose of Wegovy.
Adults on Wegovy 2.4 mg injection for heart risk or weight reduction may switch to tablets: one week after the last 2.4 mg injection, start 25 mg tablets once daily.
People on Wegovy 25 mg tablets may switch to the injection: the day after the last tablet, start 2.4 mg once weekly. For people who do not tolerate 25 mg tablets, switching to the 1.7 mg injection may be considered.
For people with type 2 diabetes on 25 mg tablets who need more weight loss, the label says switching to the 1.7 mg injection and following the injection escalation may be considered.
In Dose Track you set up Wegovy as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
