Short answer: If it has been 5 days or less since the missed dose, the Ozempic label says to take it as soon as possible. If more than 5 days have passed, skip it and take the next dose on your usual day. Doses should be at least 2 days (48 hours) apart. Then carry on once weekly.
| Situation | What the label says |
|---|---|
| Schedule | Once weekly, same day each week, any time of day (2.1) |
| Missed by 5 days or less | Take it as soon as possible (2.1) |
| Missed by more than 5 days | Skip it; take the next dose on the usual day (2.1) |
| Minimum time between doses | At least 2 days (more than 48 hours) (2.1) |
| Several weeks missed | No specific rule in the Ozempic label |
| Changing your injection day | Allowed if the last dose was 2 or more days before (2.1) |
Section 2.1 of the Ozempic prescribing information says that if a dose is missed, it should be given as soon as possible within 5 days after the missed dose. After that you go back to your regular once-weekly schedule on your usual day.
Example: your usual day is Sunday and you remember on Wednesday. That is 3 days, so under the label you would take it Wednesday and still take the next one on Sunday. Wednesday to Sunday is 4 days, which is more than the 2-day minimum.
The label says to skip the missed dose and take the next one on the regularly scheduled day. Do not double up. Using the same example, if you remember on Saturday (6 days late), you would skip it and take Sunday's dose as normal.
Section 2.1 allows the day of weekly dosing to change as long as the time between two doses is at least 2 days (more than 48 hours). The Medication Guide says the same: you may change the day as long as your last dose was 2 or more days before. The 5-day window and the 2-day gap fit together: a dose taken 5 days late still leaves 2 days before the next scheduled one.
The Ozempic label does not give a separate rule for restarting after several missed weeks. Whether to resume your usual dose or restart at a lower one is a question for your prescriber, especially if stomach side effects were a problem when you first started.
For comparison, the Wegovy label (same drug, semaglutide, at higher doses) says that if 2 or more doses in a row are missed, the dose increases should be restarted at a lower dose to reduce stomach side effects. That rule is written for Wegovy, not Ozempic, but it shows why the question is worth asking.
Semaglutide has an elimination half-life of about 1 week and stays in the blood for about 5 weeks after the last dose (label section 12.3), so one late or skipped dose does not take your level to zero.
Yes. Under section 2.1 you can move to a new day if it is at least 2 days after your last dose. After the switch, inject on the new day each week.
