Short answer: The Forzinity label dose for patients weighing at least 30 kg is 40 mg injected under the skin once daily, at the same time each day. Adults with eGFR below 30 mL/minute who are not on dialysis take 20 mg once daily. A missed dose is skipped, and the next dose is taken at the scheduled time; the label says not to double a dose.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Forzinity is approved for: Forzinity is approved to improve muscle strength in adults and children with Barth syndrome who weigh at least 30 kg. It has accelerated approval, based on improved knee extensor muscle strength; continued approval may depend on a confirmatory trial showing clinical benefit (section 1).

Forzinity is injected under the skin (subcutaneously) once daily, at the same time each day, in the abdomen (at least 2 inches from the navel) or the outer thigh. The injection site is rotated daily, avoiding skin that is tender, bruised, red or hard, and scars or stretch marks (sections 2.1 and 2.3).
It comes as a 280 mg/3.5 mL (80 mg/mL) single-patient-use vial that holds more than one dose. Vials are thrown away 8 days after first opening, and other products are never mixed in the same syringe (sections 2.3 and 3).
Adult patients or caregivers may give Forzinity after proper training, if a healthcare provider decides it is appropriate. The Instructions for Use say to use only the syringes prescribed or supplied, because the wrong syringe can lead to too much or too little medicine.
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Estimated Glomerular Filtration Rate (eGFR) (mL/minute) | Recommended Dosage |
|---|---|
| Greater than or equal to 30 mL/minute | 40 mg subcutaneously once daily |
| Less than 30 mL/minute and NOT on dialysis | 20 mg subcutaneously once daily |
The recommended dosage in patients weighing at least 30 kg is 40 mg subcutaneously once daily, given at the same time each day. There is no titration schedule.
Adults with eGFR less than 30 mL/minute who are not on dialysis take 20 mg subcutaneously once daily.
The label says there is not enough information to recommend a dose for adults with eGFR less than 30 mL/minute who are on dialysis, or for children weighing 30 kg or more who have kidney impairment.
Each vial is checked before use; Forzinity is clear, colorless to yellow, and is not used if the solution is cloudy or has particles.
Forzinity is for single-patient use only, and vials are discarded 8 days after first opening.
Forzinity is not approved for use in newborns or for intravenous use.
If a dose is missed, skip the dose and take the next dose of Forzinity at the scheduled time. Do not take a double dose.
In Dose Track you set up Forzinity as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
