Lupron Depot 3.75 mg dosing schedule

leuprolide acetate for depot suspension · monthly intramuscular injection given by a healthcare provider, 3.75 mg · Label checked

Short answer: Lupron Depot 3.75 mg is one intramuscular injection a month, given by a healthcare provider. For endometriosis it is given for up to 6 months, alone or with daily norethindrone acetate 5 mg; one retreatment course of up to 6 months uses add-back, for 12 months total at most. For fibroid anemia it is given monthly for up to 3 months with iron. The label gives no missed-dose rule.

This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.

What Lupron Depot 3.75 mg is approved for: Lupron Depot 3.75 mg is a GnRH agonist approved to manage endometriosis, including pain relief and shrinking lesions, alone or with norethindrone acetate add-back; and, with iron, to improve blood counts before surgery in women with anemia from fibroids when 3 months of hormone suppression is needed (sections 1.1 and 1.2). This page covers only the 3.75 mg monthly gynecologic label, not the 11.25 mg or prostate cancer labels.

How is Lupron Depot 3.75 mg taken?

A healthcare professional mixes the powder with the diluent in a prefilled dual-chamber syringe and gives it as a single intramuscular injection once a month (sections 2.1, 2.2 and 3).

It is not given more often than once a month, and it is not swapped with the 3-month Lupron Depot 11.25 mg: a fraction of 11.25 mg does not equal 3.75 mg, and three 3.75 mg doses do not equal 11.25 mg (section 2.1).

For fibroids, the label suggests a one-month trial of iron alone first, adding Lupron Depot if the response is not enough (section 1.2).

What is the Lupron Depot 3.75 mg dosing schedule on the label?

The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.

Table 1. LUPRON DEPOT 3.75 mg, Management of Endometriosis [Section 2.1]

Retreatment3.75 mg IM every 1 month for 1 to 6 doses6 months
Total12 MONTHS TOTAL TREATMENT DURATION

Initial treatment may use Lupron Depot with or without norethindrone acetate 5 mg daily. Retreatment uses norethindrone acetate 5 mg daily, with bone density assessed first. Treatment should not exceed 12 months because of bone density concerns. For fibroids, the dose is one IM injection every month for up to three months.

Other dosing rules on the label

  • Section 2.1

Retreatment for endometriosis is with norethindrone acetate add-back only; the label says not to re-treat with Lupron Depot 3.75 mg alone.

  • Section 1.2

Lupron Depot 3.75 mg is not indicated with norethindrone add-back for the fibroid anemia use.

  • Section 5.2

Pregnancy is excluded before starting if clinically indicated, and treatment is stopped if pregnancy occurs.

  • Section 8.3

Fertility is expected to be lower during treatment. Studies of up to 24 weeks of continuous use showed fertility suppression reversed after stopping.

  • Section 14.1

In trials, periods stopped in 74% of women after the first month and 98% after the second; normal cycles returned in 95% by the third month after treatment.

  • Section 16

    Kits are stored at 20 to 25 C (68 to 77 F), with excursions from 15 to 30 C allowed.

What does the label say about a missed dose?

  • No rule

The Lupron Depot 3.75 mg label gives no missed-dose rule. It says the injection is given by a healthcare professional once a month and not more often than once a month, but it does not say what to do if a monthly injection is late or missed.

What does the label say about switching?

  • Section 2.1

Lupron Depot 3.75 mg (1-month) and Lupron Depot 11.25 mg (3-month) release the drug differently and are not interchangeable. A fractional dose of 11.25 mg is not equivalent to 3.75 mg, and three 3.75 mg doses are not equivalent to one 11.25 mg dose.

How can Dose Track follow this schedule?

In Dose Track you set up Lupron Depot 3.75 mg as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
