Short answer: The Egrifta WR label gives one dose: 1.28 mg injected under the skin of the abdomen once daily. There is no starting dose or step-up schedule. Injection sites are rotated around the abdomen, avoiding scar tissue, bruises and the navel. One 11.6 mg vial covers daily doses for 7 days in a row. Egrifta WR and Egrifta SV are not substitutable.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Egrifta WR is approved for: Egrifta WR is approved to reduce excess abdominal fat in adults with HIV who have lipodystrophy. The label says it is not for weight loss management, its long-term heart and blood vessel safety has not been established, and there are no data that it improves how well people take their HIV medicines (section 1).

Egrifta WR is injected under the skin (subcutaneously) into the abdomen once a day. The label says to rotate injection sites to different areas of the abdomen and not to inject into scar tissue, bruises or the navel (section 2.1).
It comes as an 11.6 mg single-patient-use vial with a multiple-dose diluent. One prepared vial provides daily doses for 7 consecutive days, and any unused solution is thrown away 7 days later (sections 2.2 and 3). The label points patients to the Instructions for Use in the Injection Box.
The Patient Information says never to share an Egrifta WR syringe or needles with anyone, even if the needle has been changed.
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.

The recommended dosage of Egrifta WR is 1.28 mg subcutaneously once daily. The label gives no other dose and no step-up schedule.
There are two EGRIFTA formulations, Egrifta WR and Egrifta SV, with different recommended dosages, different numbers of vials per dose and different storage. The label says they are not substitutable, and its dosing applies only to the 11.6 mg per vial formulation.
IGF-1 levels are monitored during treatment. Stopping is considered in people whose IGF-1 stays high (for example above 3 SDS), particularly if the response is not robust.
Glucose status is checked before starting and periodically during treatment. If glucose intolerance or diabetes develops, stopping is considered in people without a clear response. People with diabetes may need changes to their diabetes medicines (section 17).
Because long-term heart safety is not established, the label says to weigh the risk and benefit of continuing in people who have not had a reduction in visceral (deep belly) fat.
Stopping Egrifta WR is considered in critically ill patients.
Egrifta WR is not used during pregnancy; the label says to stop it if pregnancy occurs (section 17). Mothers on Egrifta WR are told not to breastfeed.
The label gives no missed-dose instruction.
Egrifta WR and Egrifta SV are not substitutable. The Egrifta WR label covers only the 11.6 mg per vial formulation and points to the Egrifta SV prescribing information for the 2 mg per vial formulation.
In Dose Track you set up Egrifta WR as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
