What is survodutide?

Survodutide (BI 456906) is an investigational once-weekly injection from Boehringer Ingelheim that acts on glucagon and GLP-1 receptors. It is investigational and not FDA approved.

Also known as: BI 456906.

Common vial sizes

There is no approved survodutide product. Powder vials are usually labeled 10 mg.

Worked reconstitution example

Here is the math for a 10 mg vial mixed with 2 mL of bacteriostatic water, drawing 2.4 mg with a U-100 insulin syringe (1 mL = 100 units).

  1. Concentration. 10 mg ÷ 2 mL = 5 mg/mL
  2. Volume to draw. 2.4 mg ÷ 5 mg/mL = 0.48 mL
  3. Syringe units. 0.48 mL × 100 = 48 units
The amount here is an example to show the math, not a recommendation. How much to take is a decision for you and your prescriber.

Same amount, different water volumes

Adding more water doesn't change how much survodutide is in the vial. It only spreads it over more liquid, so each draw is bigger and easier to read.

What doses of survodutide were studied?

In the phase 2 obesity trial (le Roux, Lancet Diabetes Endocrinol 2024; 387 adults without diabetes, 46 weeks), people took 0.6, 2.4, 3.6 or 4.8 mg once weekly or placebo, with 20 weeks of dose escalation. Mean weight change at week 46 ranged from −6.2% on 0.6 mg to −14.9% on 4.8 mg, versus −2.8% on placebo.

In the phase 3 SYNCHRONIZE-1 trial (NEJM 2026; 725 adults with obesity and no diabetes, 76 weeks), doses were stepped up to 3.6 mg or 6.0 mg once weekly. Mean weight change was −12.2% and −13.0%, versus −5.4% on placebo. Gastrointestinal side effects occurred in 80.9% and 89.7% of people on survodutide, versus 47.9% on placebo. SYNCHRONIZE-2 tested the drug in adults with obesity and type 2 diabetes.

These are trial protocols, not personalized advice; confirm any dosing with your prescriber.

How to track survodutide in Dose Track

  1. Add survodutide as a custom compound. It is not in the built-in list, but a custom compound gets the same vial, units and reminder tools, and it sits next to everything else you take.
  2. Save your vial: enter the vial size and how much water you added. From then on every amount you log shows as syringe units, and the reconstitution planner suggests a water volume that lands on a clean mark.
  3. Log each shot. The app subtracts it from the vial, shows what is left, and warns you before you run out.
  4. Set reminders on your own schedule, whether that is daily, weekly or every few days.
  5. Your log stays on your phone.

Frequently asked questions

How much bacteriostatic water do I add to a 10 mg survodutide vial?+
There is no single right amount. More water makes each draw bigger and easier to measure; less water makes it smaller. For example, 2 mL in a 10 mg vial gives 5 mg/mL, so 2.4 mg is 0.48 mL, or 48 units on a U-100 insulin syringe. That amount is only an example for the math, not a recommendation. The free Dose Track reconstitution calculator works it out for any vial and amount.
How many units is 2.4 mg of survodutide?+
It depends on the concentration in your vial. At 5 mg/mL (10 mg mixed with 2 mL), 2.4 mg is 0.48 mL, which is 48 units on a U-100 insulin syringe, where 1 mL is 100 units. Divide the amount by the concentration to get mL, then multiply by 100.
What is the half-life of survodutide?+
We could not open a primary human pharmacokinetic paper for survodutide, so we do not list a half-life or time to peak here.
Can I track survodutide in Dose Track?+
Yes, as a custom compound. Survodutide is not in the built-in list, but you can add it yourself. You can then log each shot next to your other compounds, save your vial and how much water you added so every amount shows in syringe units, see how much is left in the vial, and get reminders. Your log stays on your phone.
