Short answer: With 3 mL of bacteriostatic water in a Tesamorelin + Ipamorelin, 10 mg + 3 mg vial (10 mg Tesamorelin + 3 mg Ipamorelin), every 10 units on a U-100 insulin syringe holds 333.3 mcg Tesamorelin and 100 mcg Ipamorelin. The ratio is fixed, so you cannot change one peptide without changing the others. Amounts are arithmetic on the label, not dose advice.
This blend puts tesamorelin, a growth hormone-releasing factor analogue, and ipamorelin in one vial. The two peptides are not in equal amounts in the most common version, so a draw holds more than three times as much tesamorelin as ipamorelin. Tesamorelin is FDA approved only as EGRIFTA SV for one use; a research vial mixed with ipamorelin is not that product.
Most listings are 10 mg tesamorelin + 3 mg ipamorelin (13 mg total). A 5 mg + 5 mg version also appears. The ratios differ, so read your label and use the matching table.
| Peptide | Commonly labeled | Share of vial | Guide |
|---|---|---|---|
| Tesamorelin | 10 mg | 76.9% | Tesamorelin guide |
| Ipamorelin | 3 mg | 23.1% | Ipamorelin guide |
| Peptide | Commonly labeled | Share of vial | Guide |
|---|---|---|---|
| Tesamorelin | 5 mg | 50% | Tesamorelin guide |
| Ipamorelin | 5 mg | 50% | Ipamorelin guide |
The water volume changes how big each draw is, not what is in the vial. The highlighted row is 3 mL.
| Water added | Units in vial | Tesamorelin / 10 units | Ipamorelin / 10 units |
|---|---|---|---|
| 2 mL | 200 | 500 mcg | 150 mcg |
| 3 mL | 300 | 333.3 mcg | 100 mcg |
| 5 mL | 500 | 200 mcg | 60 mcg |
| Water added | Units in vial | Tesamorelin / 10 units | Ipamorelin / 10 units |
|---|---|---|---|
| 2 mL | 200 | 250 mcg | 250 mcg |
| 3 mL | 300 | 166.7 mcg | 166.7 mcg |
| 5 mL | 500 | 100 mcg | 100 mcg |
| Units drawn | Volume | Tesamorelin | Ipamorelin |
|---|---|---|---|
| 5 units | 0.05 mL | 166.7 mcg | 50 mcg |
| 10 units | 0.1 mL | 333.3 mcg | 100 mcg |
| 20 units | 0.2 mL | 666.7 mcg | 200 mcg |
| 30 units | 0.3 mL | 1 mg | 300 mcg |
For any other vial, water volume or draw, the free peptide blend calculator does the same math for 2 to 5 peptides.
mg of that peptide ÷ mL of water. For Tesamorelin + Ipamorelin, 10 mg + 3 mg with 3 mL: Tesamorelin 10 ÷ 3 = 3.333 mg/mL, Ipamorelin 3 ÷ 3 = 1 mg/mL.amount ÷ its concentration = mL. Example: 100 mcg of Ipamorelin is 0.1 ÷ 1 = 0.1 mL.mL × 100 on a U-100 syringe: 0.1 × 100 = 10 units.mL × concentration for each other peptide: 333.3 mcg Tesamorelin.A blend is one solution, so every draw takes the same share of each peptide. In the example above, drawing 10 units for 100 mcg of Ipamorelin also gives 333.3 mcg Tesamorelin. Doubling the draw to 20 units doubles all of them, to 666.7 mcg Tesamorelin. There is no draw that changes one peptide and leaves the others where they are.
More or less water does not help either: it changes how many units each draw is, not the ratio between the peptides. If you need to set the amount of each peptide on its own, they have to be in separate vials (a stack), each with its own single-peptide calculation.
We found no published human study of Tesamorelin + Ipamorelin blend as a blend. Below is what is published for each peptide on its own. "Amounts studied" are what a study gave or reported, often by a different route; they are not personalized advice. Confirm anything about your own use with your prescriber.
FDA approved for one use, as EGRIFTA SV (tesamorelin) for injection: reducing excess abdominal fat in HIV-infected adults with lipodystrophy. The label says it is not indicated for weight loss management and that its long-term cardiovascular safety has not been established. A research vial mixed with another peptide is not the approved product. [1]
Amounts studied: The EGRIFTA SV label dose for its approved use is 1.4 mg injected under the skin once daily, and the label says to reconstitute it only with the Sterile Water for Injection supplied with it. We found no published study of tesamorelin mixed in one vial with ipamorelin.
FDA compounding list: On FDA's page of bulk drug substances that may present significant safety risks in compounding, tesamorelin is not listed (checked October 2026). [2]
Not FDA approved. It has been studied in people: a pharmacokinetic study in healthy men measured a half-life of about 2 hours, and a phase 2 trial after bowel surgery found no significant difference from placebo on its main outcome. [3][4][5]
Amounts studied: Both human studies gave it into a vein: infusions of 4.21 to 140.45 nmol/kg over 15 minutes (healthy men), and 0.03 mg/kg twice daily for up to 7 days (after bowel surgery). No published trial gave it under the skin or in a blend.
FDA compounding list: On FDA's page of bulk drug substances that may present significant safety risks in compounding, ipamorelin acetate is in category 2 for 503B outsourcing facilities (added September 29, 2023) and also in the "nominated but withdrawn" list. FDA notes a published study that "identified serious adverse events including death" when ipamorelin was given into a vein. [2]
Regulatory status. Tesamorelin is FDA approved only as EGRIFTA SV, for reducing excess abdominal fat in HIV-infected adults with lipodystrophy; a research vial mixed with ipamorelin is not that product. Ipamorelin is not FDA approved, and FDA lists ipamorelin acetate in category 2 for 503B outsourcing facilities on its page of bulk drug substances that may present significant safety risks. No tesamorelin + ipamorelin combination is FDA approved. [1][2]
