[Missed dose · Mounjaro]

I missed my Mounjaro dose. What should I do?

tirzepatide injection · Once weekly injection, any time of day · Label checked

Short answer: If it has been 4 days (96 hours) or less since the missed dose, the Mounjaro label says to take it as soon as possible. If more than 4 days have passed, skip it and take the next dose on your usual day. Two doses should be at least 3 days (72 hours) apart.

What does the Mounjaro label say about missed doses?

From the FDA label on DailyMed. Numbers in brackets are label section numbers.

Missed by more than 4 daysSkip it; take the next dose on the usual day (2.1)

What if I am 4 days late or less?

Section 2.1 of the Mounjaro prescribing information says that if a dose is missed, take it as soon as possible within 4 days (96 hours) after the missed dose. Then go back to your regular once-weekly schedule.

Example: your day is Friday and you remember on Monday (3 days late). Under the label you would inject Monday and again on Friday. Monday to Friday is 4 days, more than the 3-day minimum.

What if more than 4 days have passed?

Skip the missed dose and take the next one on your regularly scheduled day. The 4-day window is shorter than Ozempic's 5 days because tirzepatide needs at least 3 days between doses.

Can I take two Mounjaro doses close together?

No. The Medication Guide says not to take 2 doses of Mounjaro within 3 days of each other. Section 2.1 sets the same 3-day (72-hour) minimum when changing your weekly day.

What if I missed several weeks of Mounjaro?

The Mounjaro label does not give a separate rule for restarting after several missed weeks. Whether to resume your usual dose or restart at a lower one is a question for your prescriber, especially if stomach side effects were a problem when you first started.

For context, the label starts everyone at 2.5 mg once weekly and only increases in 2.5 mg steps after at least 4 weeks on a dose (section 2.1), a slow climb meant to reduce stomach side effects. Tirzepatide has an elimination half-life of about 5 days (section 12.3).

Can I change my Mounjaro injection day?

Yes, if needed, as long as the time between the two doses is at least 3 days (72 hours), per section 2.1.
