Short answer: Orforglipron (Foundayo) is a small-molecule GLP-1 pill FDA approved for weight management in April 2026, taken once daily with or without food.12 Semaglutide tablets must be taken on an empty stomach with up to 4 ounces of water.3 In ACHIEVE-3, in type 2 diabetes, orforglipron lowered HbA1c more than oral semaglutide.4
This page compares orforglipron with semaglutide, mainly the tablet forms (Rybelsus, Ozempic tablets and Wegovy tablets), since both are daily pills. For weekly semaglutide injections against tirzepatide, see Semaglutide vs tirzepatide: what did the head-to-head trial show?.

| Approval status | FDA approved April 1, 2026 for weight management; the current label has no diabetes indication12 | FDA approved: tablets for type 2 diabetes (Rybelsus, Ozempic) and weight management (Wegovy); weekly injections36 |
| Label dosing schedule | 0.8 mg daily for at least 30 days, then 2.5 mg, then 5.5 mg; may go up to 9, 14.5 or 17.2 mg, at least 30 days per step. Maximum 17.2 mg2 | Rybelsus: 3 mg daily for 30 days, then 7 mg, then 14 mg if needed. Wegovy tablets: 1.5 mg daily for 30 days, stepped up every 30 days to 25 mg36 |
| Food and water rules | Take once daily, with or without food. No clinically relevant food effect2 | Take on an empty stomach in the morning with up to 4 ounces of plain water, then wait at least 30 minutes before food, drinks or other oral medicines3 |
| Half-life | About 29 to 49 hours (label)2 | about 7 days (label: about 1 week)3 |
| Time to peak (Tmax) | 4 to 8 hours after a dose2 | About 1 hour after a tablet3 |
| Oral bioavailability | 77% after a 0.8 mg dose2 | 0.4% to 1% (Rybelsus); 1% to 2% (Ozempic tablets)3 |

| Notable label warnings | Boxed thyroid C-cell tumor warning; severe GI reactions, kidney injury, gallbladder disease, hypoglycemia with insulin or sulfonylureas. Oral contraceptives: switch or add a barrier method for 30 days after starting and after each dose increase2 | Boxed thyroid C-cell tumor warning; pancreatitis, gallbladder disease, kidney injury, diabetic retinopathy complications3 |
ACHIEVE-3 is the head-to-head trial: 1,698 adults with type 2 diabetes on metformin, randomized to orforglipron 12 or 36 mg or oral semaglutide 7 or 14 mg, once daily for 52 weeks. It was open label.4
The trial doses (12 and 36 mg) were an investigational formulation. The Foundayo label presents its trial data as equivalent commercial doses, which are numbered differently (up to 17.2 mg), so the two sets of numbers do not line up mg for mg.2
No published trial has compared orforglipron with semaglutide for weight loss. In ATTAIN-1 (3,127 adults with obesity, no diabetes, 72 weeks), orforglipron 6, 12 and 36 mg gave −7.5%, −8.4% and −11.2% average weight change vs −2.1% on placebo. Side effects led 5.3% to 10.3% to stop, vs 2.7% on placebo.5 On the label these trial doses are shown as Foundayo 5.5, 9 and 17.2 mg.2
Semaglutide is a peptide. Taken by mouth, only 0.4% to 2% of the dose reaches the blood, and it is absorbed mainly in the stomach with help from SNAC. Food, drinks and larger amounts of water all lower absorption, which is why the label asks for an empty stomach, little water and a 30-minute wait.3
Orforglipron is a small molecule, not a peptide. About 77% of a dose is absorbed, and food changed its levels only slightly, so the label lets you take it any time of day with or without food.2 Its half-life is shorter (about 29 to 49 hours vs about a week), so levels settle within about a week of daily dosing rather than several weeks.2
