Short answer: Both are semaglutide for type 2 diabetes. Rybelsus is a daily tablet taken on an empty stomach with a little water, 30 minutes before food. Ozempic is a weekly injection. By mouth only about 0.4% to 1% of the dose is absorbed, versus 89% from the injection, which is why tablet doses are much larger.12
One naming note: "Ozempic" now also names a tablet. Ozempic tablets (1.5, 4 and 9 mg) share a label with Rybelsus. This page compares Rybelsus with the Ozempic weekly injection and explains where the Ozempic and Wegovy tablets fit.
| Rybelsus (tablet) | Ozempic (injection) | |
|---|---|---|
| Active ingredient | Semaglutide, with the absorption enhancer SNAC1 | Semaglutide3 |
| How often | Once daily1 | Once weekly3 |
| How to take | Empty stomach in the morning, up to 4 oz of plain water, swallow whole, then wait at least 30 minutes before food, drinks or other pills1 | Inject under the skin of the belly, thigh or upper arm, any time of day, with or without meals3 |
| Label doses | 3 mg for 30 days (a starting dose only), then 7 mg; 14 mg if more control is needed1 | 0.25 mg for 4 weeks, then 0.5 mg; 1 mg and 2 mg if needed3 |
| Absorbed into the blood | About 0.4% to 1%1 | 89% (from Wegovy label, same molecule)2 |
| Time to peak | About 1 hour after a tablet1 | 1 to 3 days after a shot3 |
| Half-life | About 1 week1 | about 7 days3 |
| Missed dose | Skip it; take the next tablet the next day1 | Take within 5 days; after that, skip it3 |
| Approved uses | Type 2 diabetes; lowering heart risk in high-risk type 2 diabetes1 | Type 2 diabetes; heart risk with heart disease; kidney decline with chronic kidney disease3 |
| Breastfeeding | Not recommended, because SNAC passes into breast milk1 | Not restricted for the same reason; the tablet label notes semaglutide products without SNAC exist1 |
Semaglutide is a peptide, and the stomach absorbs very little of it. The tablets contain SNAC, which helps semaglutide get absorbed in the stomach, but only about 0.4% to 1% of a Rybelsus dose reaches the blood.1 Food, drinks and more water reduce that further. In the label's studies, absorption was higher with 50 mL of water than with 240 mL, and higher the longer people waited before eating.1 That is where the empty stomach, 4 oz of water and 30-minute wait come from.
Once absorbed, it is the same molecule with the same half-life of about a week, so daily tablets build up to a steady level over 4 to 5 weeks.1
These trials compared the tablet with placebo or other drugs, not with Ozempic injections.
Ozempic tablets (1.5, 4 and 9 mg) are a newer tablet with somewhat better absorption (about 1% to 2%). They give similar blood levels to Rybelsus 3, 7 and 14 mg, but the label says the two are not swapped milligram for milligram. Switching after the first 30 days goes 7 mg Rybelsus to 4 mg Ozempic tablets, or 14 mg to 9 mg.1
From the injection, the label allows people on Ozempic 0.5 mg to start Rybelsus 7 or 14 mg (or Ozempic tablets 4 or 9 mg) one week after their last shot.1 Wegovy tablets go up to 25 mg and are approved for weight and heart risk, not for blood sugar.2
