FDA rejects the vitamin workaround for compounded GLP-1s

In a Sept 18 warning letter to a compounding pharmacy, FDA took aim at GLP-1 + vitamin combinations:

  • Semaglutide + B12 and tirzepatide + niacinamide were still "essentially copies" of approved drugs
  • Citing the volume produced, FDA called the differences "pretextual", apparently a first for GLP-1 combinations, per a legal analysis
  • Prescriber justifications were repeated word for word across many records
  • Sterility and manufacturing problems were also cited

A warning letter is FDA's position, not a court ruling. FDA's April proposal to bar outsourcing facilities from bulk-compounding semaglutide, tirzepatide and liraglutide was not final as of Sept 25.

Pentagon orders testosterone screening for male troops 30+

New Pentagon clinical guidance (Sept 17) on low testosterone in male service members:

  • Screening is mandatory at 30 and older; younger members can ask to be tested
  • Treatment stays optional, even for those who qualify
  • The guidance says evidence does not support testosterone for energy, vitality, physical function or cognition; it cites benefits for sexual function and bone density
  • Doctors are told not to prescribe it to men hoping to have children soon, as it can lower sperm production
  • Women are excluded for now; their guideline is under review

Medical groups generally don't recommend automatic screening, so this departs from standard practice.

CDC: illnesses tied to compounded glutathione injections

CDC is investigating illnesses after IV or injected compounded glutathione made with a dietary supplement-grade ingredient:

  • Over 25 people sick in at least 4 states (TX, WA, FL, NY)
  • At least 3 hospitalized; 1 needed intensive care
  • First cluster: IV glutathione at a Texas wellness clinic
  • Likely cause: high endotoxin levels. Symptoms include fever, chills, body aches, dizziness and low blood pressure
  • At least 3 Texas pharmacies recalled products; FDA reminded compounders not to use supplement-grade glutathione in injectables

No glutathione product is FDA-approved in the US. Investigation ongoing; counts may change.
