Short answer: If it has been 7 days or less since the missed dose, the Repatha label says to inject it and keep your original schedule. After more than 7 days, it depends on your schedule: every-2-week users wait for the next scheduled dose; monthly users inject now and start a new monthly schedule from that date.
| Situation | What the label says |
|---|---|
| Schedule | Every 2 weeks or once a month |
| Within 7 days of the missed dose | Inject it; keep the original schedule (2.2) |
| More than 7 days, every-2-week dose | Wait for the next dose on the original schedule (2.2) |
| More than 7 days, monthly dose | Inject it; start a new schedule from today (2.2) |
| Effective half-life | About 11 to 17 days (12.3) |
Section 2.2 says that within 7 days of the missed dose, inject Repatha and resume your original schedule. The Patient Information says the same.
Example: you inject every 2 weeks on Fridays and remember the following Tuesday (4 days late). Under the label you would inject Tuesday and still take the next dose on the original Friday.
Section 2.2 says to wait until the next dose on the original schedule. With an every-2-week schedule, the next one is due within a week anyway.
Section 2.2 says to inject the dose and start a new schedule based on that date. Example: your monthly dose was due on the 1st and you remember on the 12th. Under the label you would inject on the 12th and make the 12th your new monthly date.
