Short answer: It depends on the strength printed on the vial. 0.25 mg of semaglutide is 25 units at 1 mg/mL, 12.5 units at 2 mg/mL, 10 units at 2.5 mg/mL, 5 units at 5 mg/mL on a U-100 insulin syringe. The formula is mg ÷ mg/mL × 100 = units. Units measure volume, so they change whenever the vial strength changes.
Rows are the dose steps named in the Ozempic and Wegovy labels; columns are compounded vial strengths. Each cell is units on a U-100 insulin syringe. Not personalized advice; confirm your dose, in mg and in units, with your prescriber or the dispensing pharmacy.
| Dose | 1 mg/mL | 2 mg/mL | 2.5 mg/mL | 5 mg/mL |
|---|---|---|---|---|
| 0.25 mg | 25 | 12.5 | 10 | 5 |
| 0.5 mg | 50 | 25 | 20 | 10 |
| 1 mg | 100 | 50 | 40 | 20 |
| 1.7 mg | 170 (over 1 mL) | 85 | 68 | 34 |
| 2 mg | 200 (over 1 mL) | 100 | 80 | 40 |
| 2.4 mg | 240 (over 1 mL) | 120 (over 1 mL) | 96 | 48 |
| 7.2 mg | 720 (over 1 mL) | 360 (over 1 mL) | 288 (over 1 mL) | 144 (over 1 mL) |
Cells over 100 units need more than one 1 mL syringe. Non-whole numbers fall between printed marks; see how to read insulin syringe marks.
Compounded semaglutide is dispensed already in solution, at a strength set by the pharmacy. There is no standard; FDA's 2024 alert notes one compounder may offer several concentrations. Pick the page that matches the mg/mL on your label:
| When | Label dose |
|---|---|
| Weeks 1 to 4 | 0.25 mg once weekly |
| Weeks 5 to 8 | 0.5 mg once weekly |
| Weeks 9 to 12 | 1 mg once weekly |
| Weeks 13 to 16 | 1.7 mg once weekly |
| Week 17 onward | Maintenance: 1.7 mg or 2.4 mg once weekly (2.4 mg recommended for adult weight reduction) |
For adult weight reduction, the label allows patients who have tolerated 2.4 mg for at least 4 weeks to increase to a maximum of 7.2 mg once weekly. If a dose is not tolerated during escalation, the label says to consider delaying escalation for 4 weeks.
| When | Label dose |
|---|---|
| First 4 weeks | 0.25 mg once weekly |
| After 4 weeks | 0.5 mg once weekly |
| After at least 4 more weeks, if needed | 1 mg once weekly |
| After at least 4 more weeks, if needed | 2 mg once weekly (maximum) |
The label describes 0.25 mg as a starting dose. Increases past 0.5 mg are for additional glycemic control.
FDA has received reports of overdoses, some needing hospital care, from compounded semaglutide in multi-dose vials. In the reports, patients drew more than the prescribed amount, and some prescribers miscalculated the conversion from mg to units; FDA links the errors to confusion between milligrams, milliliters and units. One example FDA gives: a 0.25 mg dose meant to be 5 units was prescribed as 25 units. The insulin syringe guide covers the syringe side of the problem.
