Rybelsus (semaglutide) dose schedule, titration chart and side effects

Novo Nordisk · Once daily tablet · Label reviewed as of

Short answer: Per the FDA label (2.2), Rybelsus starts at 3 mg once daily for days 1–30, a dose the label says is not effective for glycemic control, then 7 mg once daily for days 31–60. From day 61, the label's schedule either stays at 7 mg or increases to 14 mg if additional glycemic control is needed.

Per label section 1, Rybelsus (and Ozempic tablets) are indicated in adults with type 2 diabetes as an adjunct to diet and exercise to improve glycemic control, and to reduce the risk of major adverse cardiovascular events (cardiovascular death, non-fatal heart attack, or non-fatal stroke) in those at high risk for these events. This page summarizes the current FDA prescribing information (revised 1/2026). It is not personalized medical advice; your prescriber sets your dose.

What is the Rybelsus dose schedule?

The label's dosing schedule (label section 2.2):

Dosing schedule as written in the FDA label (label section 2.2).
PeriodDoseLabel note
Days 1–303 mg once dailyStarting (initiation) dosage; the label says it is not effective for glycemic control
Days 31–607 mg once dailyEscalation dosage
Day 61 onward7 mg once daily, or 14 mg once dailyStays at 7 mg if no additional glycemic control is needed; 14 mg if additional glycemic control is needed

Maintenance: The label maintenance dosages are 7 mg or 14 mg once daily from day 61 onward, with 14 mg used if additional glycemic control is needed (2.2). The label refers to 14 mg as the maximum recommended human dose of semaglutide tablets (13.1).

  • The label says to follow the starting and escalation schedule to reduce the risk of gastrointestinal adverse reactions (2.2).
  • The label schedule is written in days (30-day steps), not weeks (2.2).
  • The same label covers Ozempic tablets (1.5 mg, 4 mg, 9 mg), which follow a parallel schedule: 1.5 mg for days 1–30, 4 mg for days 31–60, then 4 mg or 9 mg from day 61 (2.2).
  • Rybelsus and Ozempic tablets are not substitutable mg-for-mg. Switching between them is not described during days 1–30; after that, the label pairs Rybelsus 7 mg with Ozempic tablets 4 mg, and Rybelsus 14 mg with Ozempic tablets 9 mg, starting the other product the day after stopping the first (2.1; 2.3, Table 1).
  • For people on Ozempic injection 0.5 mg, the label describes starting Rybelsus 7 mg or 14 mg once daily one week after the last injection (2.4).
  • Per the label, steady-state exposure is reached after 4 to 5 weeks of once-daily dosing (12.3).

What does the Rybelsus titration calendar look like?

Below is the label schedule laid out period by period, using the earliest timing the label allows. The label lets prescribers stay at a step longer, so a real calendar is often slower. The blank columns are for your own record.

  • The current label lists Rybelsus as 3 mg, 7 mg, and 14 mg. The 1.5 mg, 4 mg, and 9 mg strengths are branded Ozempic tablets in the same label, not Rybelsus (3; 16).
  • Per the label, there were no clinically significant differences in steady-state exposure between Rybelsus 3, 7, and 14 mg and Ozempic tablets 1.5, 4, and 9 mg, respectively (12.3).
  • The label says to store tablets at 20°C to 25°C (68°F to 77°F) in the original bottle, to protect them from moisture (16).

What are the most common Rybelsus side effects?

The label reports these rates from placebo-controlled trials (label section 6.1, Table 2). Percentages are the share of trial participants who reported each reaction.

Adverse reactions reported in ≥5% of semaglutide tablet-treated patients and more often than placebo, pool of 2 placebo-controlled trials in type 2 diabetes (label Table 2; trials used Rybelsus strengths).

When do they happen? Per the label, the majority of reports of nausea, vomiting and/or diarrhea occurred during dose escalation. Gastrointestinal adverse reactions overall occurred in 21% (placebo), 32% (7 mg), and 41% (14 mg), and led to discontinuation in 1%, 4%, and 8%, respectively (6.1).

The label also carries warnings that are not in this table (for example, a boxed warning or pancreatitis and gallbladder warnings where applicable). Read the full label linked under Sources, and report side effects to your prescriber.

How does switching to or from Rybelsus work on the label?

The label's switching table (label section 2.3, Table 1; 2.4). Switching is a prescriber decision.

What if you miss a dose of Rybelsus?

Per the label, a missed dose is skipped and the next dose is taken the following day. The label also says not to take more than one tablet per day. (label section 2.1) The full walk-through is on the Rybelsus missed-dose page.

How is Rybelsus taken?

The label describes one tablet once daily on an empty stomach in the morning, with up to 4 ounces of plain water and no other liquids, swallowed whole (not split, crushed, chewed, or dissolved). It then describes waiting at least 30 minutes before eating, drinking other beverages, or taking other oral medications. (label section 2.1)

What should you track on Rybelsus?

A simple log makes each titration step easier to talk through with your prescriber. These are the things worth writing down, and how Dose Track handles them:

  • Dose and date. Each tablet with its amount, so the titration steps above line up with what you actually took. Dose Track keeps the full history of every dose change.
  • Weight. Logged by hand or synced from Apple Health or Android Health Connect, and charted alongside your dose changes.
  • Side effects. A note of what you felt and when, so you can see whether it settles after a dose step and bring the record to your prescriber.
  • Reminders. A daily reminder at the time you choose.

Your log stays on your phone. See GLP-1 tracking in Dose Track for the full feature list.

Rybelsus questions people ask

What's the highest dose of Rybelsus?+
The highest Rybelsus dosage in the label is 14 mg once daily, used from day 61 onward if additional glycemic control is needed after 30 days on 7 mg (2.2). The label refers to 14 mg as the maximum recommended human dose of semaglutide tablets (13.1). The Ozempic tablets in the same label top out at 9 mg, which the label pairs with Rybelsus 14 mg (2.3, Table 1).
How long does it take for Rybelsus to work?+
The label does not give a single onset time. Per the label, steady-state exposure is reached after 4 to 5 weeks of once-daily dosing (12.3). In the 26-week monotherapy trial, HbA1c changed by -1.2% (7 mg) and -1.4% (14 mg) versus -0.3% with placebo at week 26 (14.2, Table 4). The label says the 3 mg starting dose is not effective for glycemic control (2.2).
Can you skip the 3 mg starter dose of Rybelsus?+
The label's schedule starts at 3 mg once daily for days 1–30, and says to follow the starting and escalation schedule to reduce the risk of gastrointestinal adverse reactions (2.2). The label notes that 3 mg is not effective for glycemic control. The one exception described is switching from Ozempic injection 0.5 mg, where Rybelsus starts at 7 mg or 14 mg (2.4). Dosing changes are for the prescriber; this is not personalized advice.
What time of day should you take Rybelsus?+
The label describes taking Rybelsus once daily in the morning on an empty stomach (2.1). The label then describes waiting at least 30 minutes before eating, drinking, or taking other oral medications. Per the label, absorption was higher with a longer post-dose fasting period in studies of 15, 30, 60, and 120 minutes (12.3).
Can you take Rybelsus with food or coffee?+
Per the label, Rybelsus is taken on an empty stomach with up to 4 ounces of water only, with no other liquids, and food, beverages, and other oral medications wait at least 30 minutes (2.1). The label cites absorption data: semaglutide absorption was higher with 50 mL than 240 mL of water, and no clinically significant difference was seen between 50 mL and 120 mL (12.3).
What happens if you miss a dose of Rybelsus?+
Per the label, a missed dose is skipped and the next dose is taken the following day (2.1). The label also says not to take more than one tablet per day, so it does not describe doubling up. The usual morning, empty-stomach routine described in the label applies to the next dose (2.1).
What is the difference between Rybelsus and Ozempic tablets?+
Both are oral semaglutide in the same FDA label. Rybelsus comes as 3, 7, and 14 mg oval tablets; Ozempic tablets come as 1.5, 4, and 9 mg round tablets (3). The label says they are not substitutable mg-for-mg and pairs Rybelsus 7 mg with Ozempic tablets 4 mg and Rybelsus 14 mg with Ozempic tablets 9 mg (2.1; 2.3). It reports similar steady-state exposure between the matched strengths (12.3).
Can you cut or crush Rybelsus tablets?+
The label says tablets are swallowed whole and are not to be split, crushed, chewed, or dissolved in any solution (2.1). The label explains that the tablets are co-formulated with SNAC, which facilitates absorption of semaglutide after oral administration, and that absorption occurs mainly in the stomach (12.3).
