Hematocrit on TRT: What It Is and the 54% Line
[Glossary · How doses act in the body]

Hematocrit

Also called Hct, HCT, packed cell volume, PCV · Reviewed

Definition: Hematocrit is the percentage of your blood volume made up of red blood cells. A result of 45 means about 45% of the blood is red cells. Testosterone can raise it, so it is one of the standard labs checked before and during testosterone therapy.

Why it matters on testosterone

The Depo-Testosterone label says androgens can stimulate red blood cell production and that hemoglobin and hematocrit should be checked periodically on long-term use. The Xyosted label says to check it about every 3 months, and to stop until it comes down if it rises. In the Xyosted study, 12 of 283 people reached a hematocrit of 55% or higher; that is 12 ÷ 283 = about 4%.

Worked example: reading a trend

Three results over a year

  1. Baseline: 44%. Month 4: 49%. Month 12: 52%.
  2. Change from baseline: 52 − 44 = 8 percentage points.
  3. Gap to the guideline stop line: 54 − 52 = 2 points.
  4. A single result can move with hydration, so the trend across draws, at the same timing, tells you more than one number.

These are example numbers to show the arithmetic, not real results or a typical course.

Common mix-ups

  • Dehydration raises it. MedlinePlus calls dehydration the most common cause of a high hematocrit, because less plasma makes red cells a bigger share.
  • Hematocrit vs hemoglobin. They are two different tests; the Depo-Testosterone label asks for both.
  • Altitude shifts the range. Living high up raises hematocrit, which is why the guideline gives a separate cutoff.

Frequently asked questions

What hematocrit is too high on TRT?+
The 2018 Endocrine Society guideline says to stop testosterone until it falls if hematocrit is above 54%, and not to start if it is above 48% (50% at high altitude). Your clinician interprets your result.
How often is hematocrit checked on testosterone?+
The guideline says before starting, 3 to 6 months in, and then yearly. The Xyosted label says about every 3 months.
Can drinking water change my hematocrit result?+
Hydration affects it. MedlinePlus says dehydration is the most common cause of a high result, so being well hydrated before a draw gives a fairer reading.
