Definition: Aspiration, in injecting, means pulling back on the syringe plunger after the needle is in, but before injecting, to check for blood in the syringe. The idea is to avoid injecting into a blood vessel. CDC says it is not necessary for vaccines, and a 2026 meta-analysis found it adds pain without a clear benefit.
What CDC and the evidence say
CDC guidance on giving vaccines says aspiration is not necessary because no large blood vessels are present at the recommended injection sites, and that a process that includes aspiration might be more painful for infants.
A meta-analysis published in Contemporary Nurse in 2026 pooled six randomized trials with 693 people getting intramuscular injections. Aspiration raised pain scores, especially in children and in deltoid shots, and added about 4.5 seconds to each injection. No serious complications were reported when aspiration was skipped. The authors say this supports guidelines that discourage routine aspiration, while noting technique should fit the person and the drug.
Pens and auto-injectors
The Ozempic, Wegovy and Mounjaro pen instructions we read have no aspiration step. With an auto-injector it is not possible at all, because there is no plunger to pull. For any product, follow the steps in its own instructions.
Common mix-ups
- Pulmonary aspiration is different. GLP-1 labels such as Ozempic and Mounjaro warn about pulmonary aspiration, which means stomach contents getting into the lungs during anesthesia or deep sedation. It has nothing to do with injection technique.
- Aspirating vs drawing up. Drawing liquid out of a vial is sometimes also called aspirating. In this entry the word means the check done after the needle is in the skin.

Do you need to aspirate before a vaccine?+
CDC says no. Its guidance says aspiration is not necessary because there are no large blood vessels at the recommended sites, and that it might be more painful for infants.
Does aspiration make an injection hurt more?+
A 2026 meta-analysis of six trials with 693 people found aspiration raised pain scores in intramuscular injections and added about 4.5 seconds, with no serious complications reported when it was skipped.
Is the pulmonary aspiration warning on GLP-1 labels about injecting?+
No. It refers to stomach contents getting into the lungs during anesthesia or deep sedation, which is a different meaning of the word.
