Short answer: For ART, the Crinone label dose is 8% gel (90 mg) vaginally once daily for progesterone supplementation, or twice daily for women with partial or complete ovarian failure, continued if pregnancy occurs up to 10 to 12 weeks. For secondary amenorrhea, 4% gel is used every other day for up to six doses, with 8% for non-responders. A missed dose is used when remembered.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Crinone is approved for: Crinone 8% is approved for progesterone supplementation or replacement as part of Assisted Reproductive Technology (ART) treatment for infertile women with progesterone deficiency. Crinone 4% is approved to treat secondary amenorrhea (periods that have stopped), and Crinone 8% is for women who did not respond to the 4% gel (Indications and Usage).

Each single-use applicator delivers 1.125 g of gel holding 45 mg (4%) or 90 mg (8%) progesterone (Description, How Supplied).
A dose increase from the 4% gel can only be done by switching to the 8% gel; using more of the 4% gel does not raise the amount of progesterone absorbed (Dosage and Administration).
Crinone is not used at the same time as other vaginal treatments; any other vaginal product is used at least 6 hours before or after Crinone (Information for Patients).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Use | Dose and schedule |
|---|---|
| ART, progesterone supplementation | Crinone 8% (90 mg) vaginally once daily |
| ART, partial or complete ovarian failure (replacement) | Crinone 8% (90 mg) vaginally twice daily |
| If pregnancy occurs in ART | Treatment may continue until placental autonomy, up to 10 to 12 weeks |
| Secondary amenorrhea | Crinone 4% vaginally every other day, up to six doses |
| Secondary amenorrhea, no response to 4% | Trial of Crinone 8% every other day, up to six doses |
A physical exam with attention to the breasts and pelvic organs, plus a Pap smear, is done before treatment.
Crinone is stopped right away if signs of thrombophlebitis, stroke, pulmonary embolism or retinal thrombosis occur or are suspected.
More Crinone than the prescribed dose is not used.
In the IVF study, Crinone 8% once daily started within 24 hours of embryo transfer and continued through Day 30 after transfer.
If a dose of Crinone is missed, it is used as soon as it is remembered. The Patient Information also says not to use more Crinone than the prescribed dose.
More on this: missed a dose of Crinone.
In Dose Track you set up Crinone as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
