Short answer: For ovulation induction, the Gonal-f RFF Redi-ject label starts at 75 IU a day under the skin for 14 days, then adjusts by up to 37.5 IU no more than every 7 days, to at most 300 IU a day. For ART it starts at 150 IU a day on cycle day 2 or 3 (225 IU at age 35 or older), up to 450 IU a day, usually for no more than 10 days.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Gonal-f RFF Redi-ject is approved for: Gonal-f RFF Redi-ject is a prefilled FSH pen approved to induce ovulation and pregnancy in oligo-anovulatory women whose infertility is functional and not due to primary ovarian failure, and to develop multiple follicles in ovulatory women as part of an Assisted Reproductive Technology (ART) cycle (section 1). This page covers the Redi-ject pen label; the Gonal-f multi-dose vial has a separate label.

Gonal-f RFF Redi-ject is a prefilled, multiple-dose pen injected under the skin of the abdomen. Doses can be set in 12.5 IU steps, and no other medicine is mixed into the pen (section 2.1).
The pen comes in 300 IU, 450 IU and 900 IU sizes. The most that can be dialed in one dose is 300 IU on the 300 IU pen and 450 IU on the 450 IU and 900 IU pens (section 3 and Instructions for Use).
The label says to take the pen out of the refrigerator at least 30 minutes before use so the injection is not cold (section 2.1).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Step | Ovulation induction | Assisted reproductive technology (ART) |
|---|---|---|
| Starting dose | 75 IU daily for 14 days in the first cycle; starting doses under 37.5 IU are not studied or recommended | 150 IU daily from cycle day 2 or 3 in women under 35; 225 IU daily in women 35 and older (gonadotropins suppressed) |
| First adjustment | After the first 14 days, up to 37.5 IU, if indicated by ovarian response | After 5 days, based on ultrasound and serum estradiol |
| Later adjustments | Up to 37.5 IU every 7 days, if indicated | Not more often than every 3 to 5 days, by no more than 75 to 150 IU each time |
| Maximum daily dose | 300 IU per day | Doses above 450 IU per day are not recommended |
| Length | In general, do not exceed 35 days | In most cases, not more than 10 days |
| Ending stimulation | Give hCG when pre-ovulatory conditions are reached; withhold hCG if OHSS risk is increased | Give hCG when follicular development is adequate; withhold hCG if OHSS risk is increased |
In later ovulation induction cycles, the starting dose and adjustments are based on the woman's past ovarian response. Dose adjustments aim to prevent multiple follicle growth and cycle cancellation.
For ovulation induction, the label encourages daily intercourse from the day before hCG until ovulation is apparent, and discourages it when OHSS risk is increased.
Ultrasound with or without serum estradiol is used to monitor follicle growth, time the hCG trigger, detect ovarian enlargement and lower the risk of OHSS and multiple pregnancy.
Before first use, the pen is refrigerated at 2 to 8 C until expiry, or kept at 20 to 25 C for up to 3 months. After the first injection it is kept refrigerated or at room temperature for up to 28 days, cap on, away from light, not frozen; unused medicine is discarded after 28 days.
The label says women should not change the dose or schedule unless their healthcare provider tells them to, and should read the dose display in bright light.
If a dose is missed or forgotten, the next dose should not be doubled, and the woman should call her healthcare provider for further dosing instructions.

In Dose Track you set up Gonal-f RFF Redi-ject as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
