Menopur Dosing Schedule: 225 IU Start, FDA Label
[Dosing schedule · Menopur]

Menopur dosing schedule

menotropins for injection · daily injection under the skin, 75 IU FSH and 75 IU LH per vial · Label checked

Short answer: The Menopur label starts IVF stimulation at 225 IU once daily under the skin, beginning on cycle day 2 or 3, after GnRH agonist suppression. The dose is adjusted after 5 days based on ultrasound and estradiol, then no more often than every 2 days and by no more than 150 IU each time. Daily doses do not exceed 450 IU, and therapy does not exceed 20 days.

This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.

What Menopur is approved for: Menopur is a gonadotropin, containing FSH and LH activity, approved for the development of multiple follicles and pregnancy in ovulatory women as part of an Assisted Reproductive Technology (ART) cycle (section 1). Before treatment, the label calls for a full gynecologic and endocrine evaluation, ruling out pregnancy and primary ovarian failure, and checking the male partner's fertility.

How is Menopur taken?

Menopur is a powder mixed with the supplied saline and injected under the skin of the abdomen, as described in the Instructions for Use (section 2.1). It is used right after mixing (section 16.2).

Each vial holds 75 IU of FSH and 75 IU of LH activity, so more than one vial may be needed for a dose (section 3 and Patient Information).

The label allows Menopur to be mixed with Bravelle (urofollitropin) in the same syringe (section 2.1).

What is the Menopur dosing schedule on the label?

The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.

First adjustmentAfter 5 days, based on ovarian response by ultrasound of follicular growth and serum estradiol
Later adjustmentsNot more frequently than every 2 days, and by no more than 150 International Units at each adjustment
Maximum daily dose450 International Units, alone or combined with Bravelle
Maximum lengthTherapy should not exceed 20 days
Ending stimulationContinue until adequate follicular development is evident, then give hCG; withhold hCG if monitoring suggests increased OHSS risk on the last day

Other dosing rules on the label

  • Section 2.2

The dosing scheme is stepwise and individualized for each woman, starting at 225 IU and staying within 450 IU a day and 20 days of therapy.

  • Section 5.10

Ultrasound with or without serum estradiol is used to monitor follicle growth, time the hCG trigger, detect ovarian enlargement and lower the risk of OHSS and multiple pregnancy.

  • Section 5.2

If there is evidence OHSS may be developing before hCG, the hCG must be withheld. Women are assessed for OHSS for at least two weeks after hCG.

  • Section 17.1

The label says women should not change the dose or schedule unless their healthcare provider tells them to.

  • Section 16.2

The powder may be stored refrigerated or at room temperature, 3 to 25 C (37 to 77 F), protected from light. Unused mixed material is discarded.

What does the label say about a missed dose?

  • Section 17.3

If a dose of Menopur is missed or forgotten, the next dose should not be doubled, and the woman should call her healthcare provider for further dosing instructions. The Patient Information says the same: call your healthcare provider right away and do not double the amount.

More on this: missed a dose of Menopur.

How can Dose Track follow this schedule?

In Dose Track you set up Menopur as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
