Short answer: The Bijuva label dose is one capsule by mouth each evening with food. Women generally start on the 0.5 mg estradiol/100 mg progesterone strength, with dose changes based on clinical response, and the label says to try tapering or stopping every 3 to 6 months. A missed evening dose is taken with food as soon as possible unless the next dose is within two hours.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Bijuva is approved for: Bijuva combines an estrogen (estradiol) and progesterone in one capsule. It is approved for a woman with a uterus to treat moderate to severe hot flashes (vasomotor symptoms) due to menopause (section 1.1). The label says timing matters, and to consider starting it in women under 60 or within 10 years of menopause onset (section 2).

A single Bijuva capsule is taken by mouth each evening with food (section 2).
There are two strengths: 0.5 mg estradiol/100 mg progesterone and 1 mg estradiol/100 mg progesterone (section 3). Cartons hold 30 capsules (section 16).
The label says to consider starting Bijuva in women under 60 or within 10 years of menopause onset, because timing affects the overall risk-benefit profile (section 2).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.

If feasible, Bijuva is stopped at least 4 to 6 weeks before surgery linked to a higher risk of blood clots, or during long immobilization.
Bijuva is stopped if a hormone-sensitive cancer is diagnosed.
FSH and estradiol blood levels have not been shown to be useful in managing moderate to severe hot flashes.
Estrogens are used at the lowest dose possible and only as long as needed, with talks about continuing every 3 to 6 months, for example.
If the evening dose is missed, it is taken with food as soon as possible, unless it is within two hours of the next evening dose (section 17 and Patient Information).
More on this: missed a dose of Bijuva.
In Dose Track you set up Bijuva as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
