Short answer: The Duavee label dose is one tablet a day, containing conjugated estrogens 0.45 mg and bazedoxifene 20 mg, for both hot flashes and osteoporosis prevention. It is swallowed whole with or without food. A missed dose is taken when remembered unless the next dose is near. Once the foil pouch is opened, the blister pack is used within 60 days.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Duavee is approved for: Duavee combines conjugated estrogens with bazedoxifene, an estrogen agonist/antagonist. It is approved for women with a uterus to treat moderate to severe hot flashes (vasomotor symptoms) of menopause and to prevent postmenopausal osteoporosis (sections 1.1, 1.2). The bazedoxifene part lowers the risk of endometrial hyperplasia from the estrogen part, so no progestin is added (section 5.3).

Duavee is taken once daily, without regard to meals, and swallowed whole (section 2.3). There is one strength: conjugated estrogens 0.45 mg with bazedoxifene 20 mg (section 3).
Tablets stay in the blister until just before use. The date the foil pouch is opened is written on the blister label, and the pack is not used if it has been open more than 60 days. With more than one pack, one pouch is opened at a time (sections 16, 17).
Women taking it to prevent osteoporosis add calcium and/or vitamin D if daily intake is not enough (section 2.4).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Item | What the label says |
|---|---|
| Hot flashes (vasomotor symptoms) | One Duavee tablet daily |
| Prevention of postmenopausal osteoporosis | One Duavee tablet daily |
| Strength | Conjugated estrogens 0.45 mg / bazedoxifene 20 mg |
| Food | Without regard to meals; swallow whole |
| Kidney impairment | Not recommended (not evaluated) |
| Women over 75 | Not studied; not recommended |
Duavee is used for the shortest duration consistent with treatment goals and risks, with periodic re-evaluation. For osteoporosis prevention alone, it is considered only for women at significant risk, after carefully considering non-estrogen medicines.
Progestins, additional estrogens and additional estrogen agonist/antagonists are not taken with Duavee.
If feasible, Duavee is stopped at least 4 to 6 weeks before surgery linked to clots and during long immobilization, and restarted only once fully mobile.
FSH and estradiol blood levels have not been shown to be useful in managing hot flashes.
If a dose of Duavee is missed, it is taken as soon as remembered unless it is almost time for the next scheduled dose. Two doses are not taken at the same time.
More on this: missed a dose of Duavee.
In Dose Track you set up Duavee as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
