Ertugliflozin: tablet strengths, label dosing and half-life

Tablet strengths · Approval status · Half-life ~16.6 hours

Short answer: Ertugliflozin has a half-life of about 16.6 hours. Levels peak about 1 hour (fasting) after a dose. People vary, so treat these as estimates.

What is ertugliflozin?

Ertugliflozin is an SGLT2 inhibitor: it makes the kidneys pass more sugar into the urine. It is sold as Steglatro tablets for type 2 diabetes, and combined with metformin (Segluromet) or sitagliptin (Steglujan).

Also known as: Steglatro, Segluromet, Steglujan.

Tablet strengths

Steglatro comes as 5 mg and 15 mg tablets.

Approval status and how it is used

The Steglatro label approves ertugliflozin, with diet and exercise, to improve blood sugar control in adults with type 2 diabetes; it is not recommended for type 1 diabetes. Label dosing: 5 mg once a day in the morning, with or without food, which may go up to 15 mg. Kidney function and fluid status are checked before starting, and it is not recommended with an eGFR under 45. A missed dose is taken as soon as possible, without doubling the next one.

The label has no boxed warning. Its warnings are ketoacidosis (which can happen with blood sugar below 250 mg/dL), lower-limb amputations (more frequent on Steglatro in the long-term heart study), fluid loss and kidney injury, genital and urinary infections including Fournier's gangrene, and low blood sugar when used with insulin or a sulfonylurea. In 26-week trials, genital yeast infections affected 9.1% (5 mg) and 12.2% (15 mg) of women vs 3.0% on placebo. Ertugliflozin half-life and time to peak

  • Half-lifeabout 16.6 hours
  • Time to peak (Tmax)about 1 hour (fasting)

The Steglatro label says peak levels come at a median of 1 hour after a dose when fasting; a high-fat meal delays this by 1 hour without changing total exposure. Nearly all of a 15 mg dose is absorbed. The mean elimination half-life in people with type 2 diabetes and normal kidney function was 16.6 hours, and steady state is reached after 4 to 6 days. It is cleared mainly by glucuronidation (UGT1A9 and UGT2B7).

Numbers shown are taken from the source above. Half-life varies from person to person.

Frequently asked questions

What is the half-life of ertugliflozin?+
The Steglatro label says peak levels come at a median of 1 hour after a dose when fasting; a high-fat meal delays this by 1 hour without changing total exposure. Nearly all of a 15 mg dose is absorbed. The mean elimination half-life in people with type 2 diabetes and normal kidney function was 16.6 hours, and steady state is reached after 4 to 6 days. What is the difference between Steglatro, Segluromet and Steglujan?+
All contain ertugliflozin. Steglatro is ertugliflozin alone; Segluromet adds metformin and Steglujan adds sitagliptin.
Can ertugliflozin cause ketoacidosis?+
Yes. The Steglatro label warns of ketoacidosis, which can happen even when blood sugar is below 250 mg/dL. Triggers include lower insulin doses, illness, eating little, a ketogenic diet, surgery and alcohol.
Can I track ertugliflozin in Dose Track?+
Yes. Ertugliflozin is in Dose Track's research library, so when you add it as a custom compound its details are filled in for you to check. You can then log each tablet next to your other compounds, set reminders on your own schedule, and see your history next to your lab results.
