Syringe Dead Space: Why Vials Run Out Early
[Glossary · Mixing and measuring]

Dead space (syringe)

Also called syringe dead space, dead volume, low dead space syringe, hub loss · Reviewed

Definition: Dead space is the liquid that stays inside a syringe, in the hub and the needle, after the plunger is pushed all the way down. It is drawn from the vial but never injected. Insulin syringes with a fixed (permanently attached) needle have very little; syringes with a detachable needle have more.

What studies show

Most published work on dead space comes from vaccine programs and harm reduction, where small losses add up. A 2015 lab study grouped syringes into three kinds: insulin syringes with fixed needles, newer low dead space designs, and standard syringes with detachable needles (high dead space). The low dead space designs sat between the other two. A 2013 review found that high dead-space syringes have on average about 10 times the dead space of low dead-space ones, though figures varied widely.

A 2023 study of COVID-19 vaccine supplies called syringe dead space a crucial factor in the number of wasted doses.

Why it matters when you track doses

Dead space is why a vial can run out before your math says it should. Each draw takes the dose plus whatever stays in the syringe. Logging every dose against the vial shows when the count and the vial stop matching.

A 2 mL vial, 0.1 mL per dose

  1. On paper: 2 mL ÷ 0.1 mL = 20 doses.
  2. Suppose each draw also loses 0.01 mL to dead space and handling. This figure is made up for the math, not measured.
  3. Each draw now takes 0.11 mL. 18 draws use 18 × 0.11 = 1.98 mL; a 19th would need 2.09 mL.
  4. Result: 18 full doses, not 20.

Common mix-ups

  • Dead space is not an air bubble. Bubbles are a drawing problem; dead space is built into the syringe.
  • Changing syringe type changes the loss. A switch from a fixed-needle insulin syringe to one with a detachable needle can mean more lost per draw.
  • It does not show on the scale. You cannot see it by reading the marks.

Frequently asked questions

What is dead space in a syringe?+
It is the liquid left in the hub and needle after the plunger is fully pressed. It comes out of the vial but is not injected.
Which syringes have the least dead space?+
In a 2015 lab study, insulin syringes with fixed needles had less than both low dead space designs and standard syringes with detachable needles.
Why does my vial run out early?+
Dead space and small handling losses on every draw add up, so a vial gives fewer doses than the volume alone suggests.
