Short answer: Segluromet is taken by mouth twice daily with meals. The starting dose is set by current medicines: from metformin, 2.5 mg ertugliflozin per tablet with a similar metformin dose; from ertugliflozin, 500 mg metformin per tablet. The maximum is 15 mg ertugliflozin and 2,000 mg metformin a day. It is not recommended with an eGFR below 45. A missed dose is taken as soon as possible, without doubling.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Segluromet is approved for: Segluromet combines ertugliflozin, an SGLT2 inhibitor, with metformin, a biguanide. It is approved, along with diet and exercise, to improve blood sugar control in adults with type 2 diabetes (section 1). It is not recommended for type 1 diabetes.

Segluromet is a tablet taken by mouth twice daily with meals. People new to metformin raise the dose gradually to reduce stomach side effects (section 2.2).
It comes as ertugliflozin 2.5 mg with metformin 500 mg or 1,000 mg, and ertugliflozin 7.5 mg with metformin 500 mg or 1,000 mg (section 3).
Kidney function and fluid status are checked before starting, and fluid loss is corrected first (section 2.1).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.

|---|---|
| Metformin HCl | SEGLUROMET tablets containing 2.5 mg ertugliflozin, with a similar total oral daily dosage of metformin HCl |

| Ertugliflozin and metformin HCl | SEGLUROMET tablets containing the same total oral daily dosage of ertugliflozin and a similar daily dosage of metformin HCl |
| Maximum recommended oral daily dosage | 15 mg ertugliflozin and 2,000 mg metformin HCl |
Dosing may be adjusted based on effectiveness and tolerability. Use is not recommended with an eGFR below 45 mL/min/1.73 m2, and it is contraindicated below 30, in end-stage kidney disease and on dialysis.
It is stopped at the time of, or before, an iodinated contrast scan with an eGFR below 60, a history of liver disease, alcoholism or heart failure, or intra-arterial contrast. eGFR is rechecked 48 hours after, and it is restarted if kidney function is stable.
It is held for at least 4 days, if possible, before surgery or procedures with long fasting, and restarted once the person is stable and eating and drinking.
With insulin or a sulfonylurea, a lower dose of those medicines may lower the risk of low blood sugar.
Tablets are stored at 20 to 25 C (68 to 77 F), with excursions from 15 to 30 C allowed, protected from moisture in a dry place.
If a dose is missed, it is taken as soon as possible, and the next dose is not doubled. The Medication Guide adds: if it is almost time for your next dose, skip the missed dose and take the medicine at the next regularly scheduled time. Do not take 2 doses of Segluromet at the same time.
More on this: missed a dose of Segluromet.
From metformin, Segluromet starts with 2.5 mg ertugliflozin tablets at a similar metformin dose. From ertugliflozin, it starts with 500 mg metformin tablets at a similar ertugliflozin dose. From both, it keeps the same ertugliflozin dose and a similar metformin dose.
In Dose Track you set up Segluromet as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
