Short answer: Baqsimi is a single rescue dose for severe low blood sugar, not a scheduled medicine. The label gives 3 mg as one press of the device into one nostril, for adults and children aged 1 and older; it does not need to be inhaled. Emergency help is called right away, and if there is no response after 15 minutes, another 3 mg dose from a new device may be given.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Baqsimi is approved for: Baqsimi is approved to treat severe low blood sugar (severe hypoglycemia) in adults and children aged 1 year and older with diabetes (section 1).

Baqsimi is given into one nostril as soon as severe low blood sugar is recognized, by inserting the tip and pressing the plunger all the way in until the green line no longer shows. The dose does not need to be inhaled. The device is not tested or primed beforehand (section 2.1).
Each device holds one 3 mg dose of glucagon as a white powder and cannot be reused. Because a person with severe hypoglycemia needs others to help, the label says the people around them are told about Baqsimi and its Instructions for Use (sections 2.1 and 3).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Who | Dose | If no response after 15 minutes |
|---|---|---|
| Adults and children aged 1 year and older | 3 mg, one actuation into one nostril | An additional 3 mg from a new device may be given while waiting for emergency help |
Emergency help is called immediately after the dose is given.
Once the person responds, they are given carbohydrates by mouth to restore liver glycogen and prevent low blood sugar coming back.
The plunger is not pushed and the device is not tested before use. Each device is used once and thrown away.
Baqsimi is stored up to 86 F (30 C) in its shrink-wrapped tube until use. If the tube has been opened, it may have been exposed to moisture and may not work as expected.
A used Baqsimi is replaced right away so a new one is on hand.
The Baqsimi label has no missed-dose rule because Baqsimi is not taken on a schedule; it is a single rescue dose used when severe low blood sugar happens. The label's only repeat-dose rule is that if there is no response after 15 minutes, another 3 mg dose from a new device may be given while waiting for emergency help (section 2.2).
In Dose Track you set up Baqsimi as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
