Short answer: Prempro is one tablet by mouth once daily, in four strengths from 0.3 mg/1.5 mg to 0.625 mg/5 mg. Premphase is a 28-day pack: one maroon 0.625 mg conjugated estrogens tablet on days 1 to 14, then one light-blue tablet with 0.625 mg estrogens plus 5 mg medroxyprogesterone on days 15 to 28. The label calls for the lowest effective dose for the shortest duration.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Prempro is approved for: Prempro and Premphase combine conjugated estrogens (CE) with the progestin medroxyprogesterone acetate (MPA). They are approved for a woman with a uterus to treat moderate to severe hot flashes and moderate to severe vulvar and vaginal atrophy due to menopause, and to prevent postmenopausal osteoporosis (sections 1.1 to 1.3). Prempro is one tablet daily; Premphase gives CE alone on days 1 to 14 and CE plus MPA on days 15 to 28 (section 2).

Estrogen, alone or with a progestin, is used at the lowest effective dose for the shortest duration consistent with treatment goals and risks, and women are re-evaluated periodically to see if treatment is still needed.
When prescribing only for vulvar and vaginal atrophy, the label says topical vaginal products should be considered.
When prescribing only to prevent osteoporosis, therapy is considered only for women at significant risk, and non-estrogen medicines should be carefully considered.
If feasible, estrogens are stopped at least 4 to 6 weeks before surgery linked to a higher risk of blood clots, or during long immobilization.
Yearly pelvic exam, breast exam and mammogram are advised unless the healthcare provider says otherwise, and vaginal bleeding on treatment is reported right away.
The Patient Information says: if you miss a dose, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your normal schedule. Do not take 2 doses at the same time.
More on this: missed a dose of Prempro.
In Dose Track you set up Prempro as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
