Short answer: The Intrarosa label dose is one vaginal insert, containing 6.5 mg prasterone, once daily at bedtime, placed with the applicator that comes in the box. Each applicator is used once and thrown away. There is a single strength and no step-up. The label gives no missed-dose instruction. It can be stored at room temperature or in the refrigerator.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Intrarosa is approved for: Intrarosa is a steroid vaginal insert approved to treat moderate to severe dyspareunia (pain during sex), a symptom of vulvar and vaginal atrophy, due to menopause (section 1). Prasterone, also called DHEA, is an inactive steroid that the body converts into androgens and estrogens; how it works for vaginal atrophy is not fully established (section 12.1).

One Intrarosa vaginal insert is placed in the vagina once daily at bedtime using the provided applicator (section 2). Each insert holds 6.5 mg of prasterone in a solid fat bullet shape about 28 mm long (section 3).
The Instructions for Use say to empty the bladder and wash hands first, activate the applicator by pulling back the plunger, load the flat end of the insert, slide it in as far as is comfortable while lying or standing, and press the plunger.
Each applicator is for one time use only. An insert that falls on an unsanitary surface is replaced with a new one (Instructions for Use).

Any postmenopausal woman with undiagnosed, persistent or recurring genital bleeding is evaluated for the cause before Intrarosa is considered.
Intrarosa has not been studied in women with a history of breast cancer, and estrogen is contraindicated with a known or suspected history of breast cancer.
Safety and effectiveness have not been established in children; Intrarosa is indicated only for postmenopausal women.
The Intrarosa label gives no missed-dose instruction in the prescribing information, Patient Information or Instructions for Use. It says to use Intrarosa exactly as the healthcare provider says. Ask your prescriber or pharmacist what to do if a dose is missed.
In Dose Track you set up Intrarosa as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
