What is menotropins?

Menotropins (human menopausal gonadotropin, hMG) is a purified mix of FSH and LH activity taken from urine. It is sold as Menopur (Ferring), a powder mixed with saline and injected under the skin to grow multiple egg follicles during IVF.

Vial strength

Each Menopur vial holds 75 IU of FSH and 75 IU of LH activity as a powder, mixed with the supplied saline and used right away. More than one vial may be needed for a dose. The label allows mixing it with Bravelle (urofollitropin) in the same syringe.

Approval status and how it is used

The Menopur label approves menotropins to grow multiple follicles and achieve pregnancy in ovulatory women having assisted reproduction (ART). Label dosing starts at 225 IU once a day under the skin on cycle day 2 or 3, after GnRH agonist suppression. The dose is adjusted after 5 days by ultrasound and estradiol, then no more often than every 2 days and by no more than 150 IU at a time, up to 450 IU a day and no more than 20 days of treatment. It is for physicians experienced in infertility care.

The label has no boxed warning. Warnings cover ovarian hyperstimulation syndrome (OHSS), abnormal ovarian enlargement, lung and blood vessel complications, ovarian torsion, multiple births, and ectopic pregnancy and miscarriage. In IVF and ICSI trials of 434 women, belly pain (6.7%), headache (6.2%) and OHSS (6.2%) were the most common side effects.
