How to store Lyumjev (insulin lispro-aabc)

Short answer: Keep unopened Lyumjev in the fridge at 36-46°F (2-8°C), in its carton, until the expiration date. At room temperature below 86°F (30°C) it lasts 28 days in total, counting unopened and in-use time together. In-use vials may be refrigerated or not; in-use pens and cartridges are not refrigerated. What does the Lyumjev label say?

Section 16.2: “Refrigerate unopened LYUMJEV vials, pens, and cartridges between 36°F to 46°F (2°C to 8°C) until time of use and keep in the original carton to protect from light. Do not freeze or use LYUMJEV if it has been frozen. Do not expose to direct heat. Discard opened or unopened LYUMJEV vials, pens, and cartridges stored at room temperature below 86°F (30°C) after 28 days.”

Why is it 28 days in total?

Because the footnote counts all room-temperature time together. A pen that sat out unopened for a week has three weeks left once you start it, not 28 days. Keeping spares in the fridge until needed keeps the full 28 days for when the pen is opened. The Humalog label, also insulin lispro, uses the same total-time rule.

What do the Instructions for Use add?

  • Pens in use: “Store the Pen you are currently using at room temperature [up to 86°F (30°C)]. Keep away from heat and light.” Throw it away after 28 days, even if insulin is left.
  • “Do not store the Pen with the needle attached to prevent leaking, blocking the needle, and air from entering the Pen.”
  • Vials in use: kept in the fridge or below 86°F (30°C) for up to 28 days, then thrown away even if insulin is left.
  • Pump: change Lyumjev U-100 in the reservoir at least every 9 days, or as the pump manual says if that is shorter, or after exposure to temperatures above 98.6°F (37°C) (section 16.2). U-200 is not used in a pump.
  • Diluted Lyumjev for IV use: up to 4 days refrigerated at 36-46°F (2-8°C), or up to 12 hours at 68-77°F (20-25°C).

How do you travel with it?

An in-use Lyumjev pen travels at room temperature for the rest of its 28 days. Unopened spares start using up their 28 days as soon as they leave the fridge. Keep them off direct contact with an ice pack, since frozen Lyumjev is not used.

  • TSA allows medically necessary liquids over 3.4 oz in carry-on bags; declare them at the checkpoint for inspection.
  • Unused syringes and pen needles are allowed when they travel with the injectable medicine.
  • Ice packs and gel packs that keep medicine cool may go through frozen, partly frozen or melted.
  • Keep medicine in your carry-on, not checked luggage, where you cannot control the temperature. Do not let it touch an ice pack directly, since most labels say do not freeze.
