How to store Fiasp (insulin aspart)

Quoted from the US label · Unopened vs in use · Travel · Sources · Reviewed

Short answer: Keep unopened Fiasp in the fridge at 36-46°F (2-8°C) until the expiration date, or up to 86°F (30°C) for 28 days. Once in use, vials and FlexTouch pens last 28 days in the fridge or up to 86°F; PenFill cartridges last 28 days and are not refrigerated. PumpCart cartridges have shorter limits. Never use Fiasp that froze. Source: Fiasp label, section 16.2.

What does the Fiasp label say?

Section 16.2: “Unused FIASP vials should be stored between 2° to 8°C (36° to 46°F) in a refrigerator, but not in or near a freezing compartment. FIASP should not be exposed to excessive heat or light and must never be frozen. Do not use FIASP if it has been frozen. FIASP should not be drawn into a syringe and stored for later use.”

Table footnotes: for a vial used to fill a pump, “The total in use time is 28 days, including 6 days in the pump reservoir.” For PumpCart: “The maximum time at room temperature is 18 days including 4 days in the pump.” Section 16.2 also says to keep the cap on the pen to protect it from light, to remove the needle after each injection and store the pen without a needle attached, and to keep unused vials, pens and cartridges in the carton.

Is it the same as NovoLog?

No. Both are insulin aspart, but they have separate labels with different storage tables. The Fiasp table allows an in-use FlexTouch pen to be kept in the fridge for its 28 days; the NovoLog table says in-use FlexPen and FlexTouch pens are not refrigerated. Follow the label of the product you have.

What do the Instructions for Use add?

  • FlexTouch pen in use: “Store the Pen you are currently using without the needle attached at room temperature below 86°F (30°C) or in the refrigerator at 36°F to 46°F (2°C to 8°C) for up to 28 days.” It is thrown away after 28 days, even if insulin is left and the expiration date has not passed.
  • PenFill cartridges before use: stored in the fridge. “If FIASP is stored mistakenly outside of refrigeration between 47°F (9°C) to 86°F (30°C) prior to first use, it should be used within 28 days or thrown away.” In use: room temperature below 86°F (30°C) for up to 28 days, do not refrigerate.
  • Vials: opened vials can be kept in the fridge or below 86°F (30°C), and are thrown away after 28 days “(including 6 days pump in-use time)”.
  • PumpCart: in use, do not refrigerate; keep below 98.6°F (37°C) for up to 4 days, and replace at least every 4 days or as the pump manual says if that is shorter.

What about Fiasp in a pump or IV bag?

Section 16.2: change Fiasp in the pump reservoir at least every 6 days, or as the pump manual says if that is shorter, and after exposure to temperatures above 98.6°F (37°C). Infusion bags prepared for IV use are stable at 68-77°F (20-25°C) for 24 hours.

How do you travel with it?

An in-use Fiasp pen or vial travels at room temperature, up to 86°F (30°C), for the rest of its 28 days. Unopened spares can stay cold until needed; once out of the fridge they have 28 days. Keep them off direct contact with an ice pack, since frozen Fiasp is not used.

  • TSA allows medically necessary liquids over 3.4 oz in carry-on bags; declare them at the checkpoint for inspection.
  • Unused syringes and pen needles are allowed when they travel with the injectable medicine.
  • Ice packs and gel packs that keep medicine cool may go through frozen, partly frozen or melted.
  • Keep medicine in your carry-on, not checked luggage, where you cannot control the temperature. Do not let it touch an ice pack directly, since most labels say do not freeze.
