Short answer: The Natesto label has no missed-dose instructions. It describes 11 mg, one pump in each nostril, three times a day: morning, afternoon and evening, 6 to 8 hours apart. Testosterone from Natesto has a half-life of 10 to 100 minutes, so each dose covers hours, not days. Ask your prescriber how to handle a missed dose.
| Situation | What the label says |

| Missed-dose rule | None in the label |
| Half-life | 10 to 100 minutes; peak about 40 minutes after a dose (12.3) |
| After each dose | No blowing the nose or sniffing for 1 hour (2.2) |
No. We checked the full Natesto prescribing information and Patient Information on DailyMed. Neither has a missed-dose or catch-up instruction.
Section 2.2 says Natesto is used three times daily, once in the morning, once in the afternoon and once in the evening, 6 to 8 hours apart, preferably at the same time each day. Each dose is one full pump in each nostril.
Natesto is short-acting. Section 12.3 says the highest level comes about 40 minutes after a dose and the half-life ranges from 10 to 100 minutes. That is why it is used three times a day. A missed dose means lower levels for part of that day. With a testosterone shot given every 2 to 4 weeks, one late dose shifts levels over several days instead.
The label does not describe a double dose or doses closer together than 6 to 8 hours. Section 2.2 says the dose is one full press of the pump in each nostril. Ask your prescriber before changing the amount or the spacing.
It can. Section 2.1 says total testosterone is checked periodically, starting as soon as one month after starting Natesto. Treatment is stopped if levels are consistently above 1050 ng/dL, and another treatment is considered if they are consistently below 300 ng/dL. With a half-life measured in minutes, a result depends on when the last dose was taken, so whoever reads your labs needs to know about missed doses.
Section 2.4 says Natesto is stopped temporarily during an episode of severe rhinitis, until the symptoms clear. If the severe symptoms persist, the label recommends a different testosterone treatment. Section 8.9 adds that testosterone levels were 21 to 24% lower in men with symptomatic allergic rhinitis.
