Definition: Cmax is the highest concentration of a drug measured in the blood after a dose. It is read straight from measured blood levels, not estimated between samples. It comes paired with Tmax, the time it takes to get there, and is often compared with the lowest level, the trough.
Why it matters when you track doses
The peak is the highest exposure in each dosing interval. Two plans with the same weekly total can have different peaks: one big dose makes a higher Cmax and a lower trough than the same amount split into smaller, more frequent doses. That is the main reason people compare weekly with twice-weekly injections.
The FDA lists Cmax, Cmin (the lowest level), Ctrough, Cav (the average) and the fluctuation, (Cmax − Cmin) ÷ Cav, as the standard steady-state measures in multiple-dose studies.
Worked example: peak vs trough on the Xyosted label
Week 12 of weekly Xyosted, mean values from the label
- Cmax (peak) = 790 ng/dL. Cmin (lowest) = 436 ng/dL. Cavg (average over the week) = 553 ng/dL.
- Peak-to-trough ratio: 790 ÷ 436 = about 1.8. The peak was about 1.8 times the low point.
- Fluctuation: (790 − 436) ÷ 553 = 354 ÷ 553 = about 64% of the average.
- These are group means. The label reports individual Cmax values from 389 to 1,410 ng/dL.
Common mix-ups
- A lab result is rarely your Cmax. A blood draw catches one moment. Unless it was taken near Tmax, it is not your peak.
- Higher Cmax is not better. It is just the top of the range. What matters clinically depends on the drug.
- Units differ. Testosterone is reported in ng/dL in the US and nmol/L in many other countries. Check the unit before comparing.

What does Cmax mean on a drug label?+
It is the highest blood concentration measured after a dose, usually reported as an average from a study group. It tells you how high the peak goes, not when.
Does splitting a dose lower Cmax?+
In general, the same weekly total given in smaller, more frequent doses gives a lower peak and a higher trough. How much depends on the drug and its half-life.
