Short answer: The Steglujan label starts at 5 mg ertugliflozin with 100 mg sitagliptin once daily in the morning, with or without food. In people tolerating it, the dose may go to 15 mg/100 mg once daily for more blood sugar control. It is not recommended with an eGFR below 45 and is held at least 4 days before surgery with long fasting. A missed dose is taken as soon as possible, without doubling.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Steglujan is approved for: Steglujan combines ertugliflozin, an SGLT2 inhibitor, with sitagliptin, a DPP-4 inhibitor. It is approved, along with diet and exercise, to improve blood sugar control in adults with type 2 diabetes (section 1). It is not recommended for type 1 diabetes, and it has not been studied in people who have had pancreatitis.

Steglujan is a tablet taken by mouth once daily in the morning, with or without food (section 2.2).
It comes as ertugliflozin 5 mg/sitagliptin 100 mg and ertugliflozin 15 mg/sitagliptin 100 mg (section 3).
Kidney function and fluid status are checked before starting, and fluid loss is corrected first (section 2.1).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Step | Steglujan dose |
|---|---|
| Recommended starting dosage | 5 mg ertugliflozin/100 mg sitagliptin orally once daily, taken in the morning, with or without food |
| Switching from ertugliflozin | The dosage of ertugliflozin can be maintained |
| For additional glycemic control, in patients tolerating Steglujan | May be increased to 15 mg ertugliflozin/100 mg sitagliptin orally once daily |
Use is not recommended with an eGFR below 45 mL/min/1.73 m2, and it is contraindicated below 30, in end-stage kidney disease and on dialysis.
It is held for at least 4 days, if possible, before surgery or procedures with long fasting, and restarted once the person is stable and eating and drinking.
With insulin or a sulfonylurea, a lower dose of those medicines may lower the risk of low blood sugar.
Tablets are stored at 20 to 25 C (68 to 77 F), with excursions from 15 to 30 C allowed, protected from moisture in a dry place.
If a dose is missed, it is taken as soon as possible, and the next dose is not doubled. The Medication Guide adds: if it is almost time for your next dose, skip the missed dose and take the medicine at the next regularly scheduled time. Do not take 2 doses of Steglujan at the same time.
More on this: missed a dose of Steglujan.
People switching from ertugliflozin can keep the same ertugliflozin dose in Steglujan.
In Dose Track you set up Steglujan as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
