Short answer: On the Sprycel label, adults with chronic phase CML start at 100 mg once a day; adults with accelerated or blast phase CML or Ph+ ALL start at 140 mg once a day. Children's doses go by weight, from 40 mg to 100 mg. Tablets are swallowed whole, with or without a meal. If a dose is missed, the next dose is taken at its regular time, not doubled.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Sprycel is approved for: Sprycel is a kinase inhibitor approved for adults with newly diagnosed Philadelphia chromosome-positive (Ph+) chronic myeloid leukemia (CML) in chronic phase; adults with chronic, accelerated or blast phase Ph+ CML resistant to or intolerant of prior therapy including imatinib; adults with Ph+ acute lymphoblastic leukemia (ALL) resistant to or intolerant of prior therapy; and children 1 year and older with chronic phase Ph+ CML or, with chemotherapy, newly diagnosed Ph+ ALL (section 1).

Sprycel is taken by mouth once a day, with or without a meal, in the morning or the evening. Tablets are swallowed whole and not crushed, cut or chewed (section 2.1).
Grapefruit juice and strong CYP3A4 inhibitors or inducers are avoided. H2 blockers and proton pump inhibitors are not taken with Sprycel; antacids may be used at least 2 hours before or after a dose (sections 2.3 and 7.1).
Doses are interrupted, lowered or raised based on blood counts, side effects and response, following the label's tables (sections 2.4 and 2.5).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Patient group | Once-daily dose |
|---|---|
| Adults, chronic phase CML | 100 mg |
| Adults, accelerated phase CML, myeloid or lymphoid blast phase CML, or Ph+ ALL | 140 mg |
| Children 10 to less than 20 kg | 40 mg |
| Children 20 to less than 30 kg | 60 mg |
| Children 30 to less than 45 kg | 70 mg |
| Children at least 45 kg | 100 mg |
Children's doses are recalculated every 3 months as weight changes. Tablets are not recommended under 10 kg. In children with Ph+ ALL, Sprycel starts on or before day 15 of induction chemotherapy and continues for 2 years.
| Group | Starting dose | Escalation |
|---|---|---|
| Adults, chronic phase CML | 100 mg | 140 mg once daily |
| Adults, advanced phase CML and Ph+ ALL | 140 mg | 180 mg once daily |
| Children with CML | 40 mg | 50 mg |
| Children with CML | 60 mg | 70 mg |
| Children with CML | 70 mg | 90 mg |
| Children with CML | 100 mg | 120 mg |
Considered when there is no blood (hematologic) or genetic (cytogenetic) response at the starting dose. Escalation is not recommended for children with Ph+ ALL on chemotherapy.
| Current Sprycel dose | Dose to consider with a strong CYP3A4 inhibitor |
|---|---|
| 140 mg daily | 40 mg daily |
| 100 mg daily | 20 mg daily |
| 70 mg daily | 20 mg daily |
| 60 mg or 40 mg daily | Consider interrupting Sprycel until the inhibitor is discontinued |
If a strong CYP3A4 inhibitor cannot be avoided. Clinical data are not available for these reduced doses. After the inhibitor stops, about 1 week of washout is allowed before going back up.
In adult chronic phase CML, if neutrophils fall below 0.5 x 10^9/L or platelets below 50 x 10^9/L, Sprycel is stopped until counts recover, then resumed at the starting dose if recovery takes 7 days or less. A second episode resumes at 80 mg daily; a third at 50 mg (newly diagnosed) or Sprycel is stopped (resistant or intolerant) (Table 3).
In advanced phase CML or Ph+ ALL, recurring low counts unrelated to leukemia lead to 100 mg daily (second episode) or 80 mg daily (third episode) (Table 3).
For a severe non-blood side effect, Sprycel is withheld until it resolves or improves, then may resume at a reduced dose.
In studies, adults and children with chronic phase CML stayed on Sprycel until the disease progressed or it was no longer tolerated. The effect of stopping after a response has not been established. Children with Ph+ ALL received it for up to 2 years.
Stored at 20 to 25 C (68 to 77 F), with excursions from 15 to 30 C (59 to 86 F) allowed. Crushed or broken tablets are handled with latex or nitrile gloves, and pregnant people avoid exposure to them.
If a dose of Sprycel is missed, the next scheduled dose is taken at its regular time. Two doses are not taken at the same time. The Patient Information says the same.
More on this: missed a dose of Sprycel.
In Dose Track you set up Sprycel as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
