[Dosing schedule · Jardiance]

Jardiance dosing schedule

empagliflozin · once-daily oral tablet, 10 mg or 25 mg · Label checked

Short answer: The Jardiance label dose is 10 mg by mouth once daily in the morning, with or without food, for heart failure, kidney disease, heart risk and type 2 diabetes. For extra blood sugar control only, it may go up to 25 mg once daily in people tolerating 10 mg. It is held for at least 3 days before surgery when possible. A missed dose is taken as soon as possible, without doubling.

This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.

What Jardiance is approved for: Jardiance is an SGLT2 inhibitor approved to lower the risk of cardiovascular death and heart failure hospitalization in adults with heart failure; to lower the risk of kidney decline, end-stage kidney disease, cardiovascular death and hospitalization in adults with chronic kidney disease at risk of progression; to lower the risk of cardiovascular death in adults with type 2 diabetes and heart disease; and, with diet and exercise, to improve blood sugar in adults and children 10 and older with type 2 diabetes (section 1). It is not for blood sugar control in type 1 diabetes.

How is Jardiance taken?

Jardiance is a tablet taken by mouth once daily in the morning, with or without food. It comes in 10 mg and 25 mg tablets (sections 2.2 and 3).

Before starting, kidney function and fluid status are assessed, and fluid loss is corrected first. Use for blood sugar control is not recommended with an eGFR below 30 mL/min/1.73 m2 (section 2.1).

Jardiance is held for at least 3 days, if possible, before surgery or procedures with prolonged fasting, and restarted once the person is stable and eating and drinking again (section 2.3).

What is the Jardiance dosing schedule on the label?

The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.

Other dosing rules on the label

  • Section 2.1

Kidney function is assessed before starting and as clinically indicated. Fluid status is assessed, and fluid loss is corrected before starting.

  • Section 2.3

Jardiance is withheld for at least 3 days, if possible, before surgery or procedures with prolonged fasting, and resumed when the person is clinically stable and eating and drinking again.

  • Section 5.4

When starting Jardiance with a sulfonylurea or insulin, a lower dose of the sulfonylurea or insulin may be considered to reduce the risk of low blood sugar.

  • Section 7

With lithium, serum lithium is checked more often when Jardiance is started or its dose changes, since lithium levels may fall.

  • Section 8.6

Trials did not enroll adults with an eGFR below 20 or on dialysis, but people already enrolled were not required to stop if eGFR fell below 20 or dialysis started.

  • Section 8.7

Jardiance may be used with liver impairment.

What does the label say about a missed dose?

  • Section 2.4

If a dose is missed, it is taken as soon as possible, and the next dose is not doubled. The Medication Guide adds: if it is almost time for the next dose, skip the missed dose and take the medicine at the next regularly scheduled time; do not take two doses at the same time.

More on this: missed a dose of Jardiance.

How can Dose Track follow this schedule?

In Dose Track you set up Jardiance as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
