Short answer: Invokamet is one tablet twice daily with meals; Invokamet XR is two tablets once daily with the morning meal. The start depends on current medicines, usually canagliflozin 100 mg a day. Canagliflozin can go up to 300 mg a day and metformin up to 2,000 mg a day, raised gradually. Starting is not recommended with an eGFR below 45. A missed dose is taken as soon as possible, without doubling.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Invokamet is approved for: Invokamet combines canagliflozin, an SGLT2 inhibitor, with metformin, a biguanide. Along with diet and exercise, Invokamet and Invokamet XR are approved to improve blood sugar control in adults and children 10 years and older with type 2 diabetes (section 1). In adults with type 2 diabetes the canagliflozin part also lowers the risk of major heart events in established heart disease, and of kidney failure, heart death and heart failure hospital stays in diabetic kidney disease with albuminuria above 300 mg/day. It is not recommended for type 1 diabetes.

Invokamet is one tablet by mouth twice daily with meals. Invokamet XR is two tablets once daily with the morning meal, swallowed whole and never crushed, cut or chewed (section 2.2).
The starting dose is set by the current regimen (Table 1) and by kidney function (Table 2) (section 2.2).
Kidney function is checked before starting, and fluid status is checked and corrected first (sections 2.1 and 5.4).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Current regimen | Recommended total daily dosage (Invokamet or Invokamet XR) |
|---|---|
| Not treated with either canagliflozin or metformin HCl | Canagliflozin 100 mg and metformin HCl 1,000 mg |
| Metformin HCl | Canagliflozin 100 mg and the nearest appropriate total daily dosage of metformin HCl. For patients taking an evening dosage of metformin HCl extended-release tablets, skip the last dose before starting the following morning. |
| Canagliflozin | The same total daily dosage of canagliflozin and a total daily dosage of metformin HCl 1,000 mg |
| Canagliflozin and metformin HCl | The same total daily dosage of canagliflozin and the nearest appropriate total daily dosage of metformin HCl |
| eGFR (mL/min/1.73 m2) | Recommended dosage |
|---|---|
| 45 to less than 60 | The maximum total daily dosage of canagliflozin is 100 mg. |
| 30 to less than 45 | Assess the benefit risk of continuing. The maximum total daily dosage of canagliflozin is 100 mg. |
| Less than 30 | Contraindicated. If eGFR falls below 30 during treatment, discontinue. |
For more blood sugar control, canagliflozin may go to a maximum of 300 mg a day (150 mg twice daily for Invokamet) in people tolerating 100 mg, and metformin to a maximum of 2,000 mg a day, raised gradually to reduce stomach side effects.
Starting Invokamet is not recommended with an eGFR below 45 mL/min/1.73 m2 because of the metformin.
With a UGT inducer such as rifampin, phenytoin, phenobarbital or ritonavir, canagliflozin is raised to 200 mg a day in people tolerating 100 mg; with an eGFR of 60 or more the maximum stays 300 mg, and below 60 the maximum is 200 mg.
It is stopped at the time of, or before, an iodinated contrast scan with an eGFR below 60, a history of liver disease, alcoholism or heart failure, or intra-arterial contrast. eGFR is rechecked 48 hours after.
It is held at least 3 days, if possible, before surgery or procedures with long fasting.
Tablets are stored at 20 to 25 C (68 to 77 F), with excursions from 15 to 30 C allowed.
If a dose is missed, it is taken as soon as possible, and the next dose is not doubled. The Medication Guide adds: if it is almost time for your next dose, skip the missed dose and take the medicine at the next regularly scheduled time. Do not take 2 tablets of Invokamet (or more than 2 tablets of Invokamet XR) at the same time.
More on this: missed a dose of Invokamet.
From metformin alone, the start is canagliflozin 100 mg a day plus the nearest metformin dose; people on evening extended-release metformin skip that last dose before starting the next morning. From canagliflozin alone, the same canagliflozin dose plus metformin 1,000 mg a day.
In Dose Track you set up Invokamet as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
