Short answer: Denosumab has a half-life of about 25.4 days. People vary, so treat these as estimates.
Denosumab is an antibody that blocks RANK ligand, a signal that switches on the cells that break bone down. Blocking it slows bone loss and raises bone density. It is sold as Prolia (Amgen), a shot given under the skin every 6 months by a healthcare provider, for osteoporosis and for bone loss from some cancer hormone treatments.
Also known as: Prolia.
Prolia comes as a 60 mg single-dose prefilled syringe, injected under the skin of the upper arm, upper thigh or belly once every 6 months.
The Prolia label approves denosumab for women after menopause with osteoporosis at high risk of fracture, to build bone in men with osteoporosis at high risk of fracture, for steroid-caused osteoporosis at high risk of fracture, and to build bone in men on hormone-lowering treatment for prostate cancer and women on aromatase inhibitors for breast cancer who are at high risk of fracture. Label dosing is 60 mg every 6 months, given by a healthcare provider, with calcium 1,000 mg and at least 400 IU of vitamin D every day. A missed dose is given as soon as possible, and later shots are every 6 months from that date.
Prolia has a boxed warning: severe, sometimes fatal, low blood calcium in people with advanced kidney disease. Other warnings cover jaw bone death, unusual thigh fractures, serious infections including skin infections, and multiple spine fractures reported after stopping Prolia; the label says people who stop should move to another bone drug. In the 3-year trial of 7,808 women, the most common side effects were back pain (34.7% vs 34.6% on placebo), arm or leg pain and high cholesterol. Full numbers are on the Prolia side effects, Prolia dosing schedule and missed Prolia dose pages.
The Prolia label gives a mean half-life of 25.4 days, with blood levels falling over 4 to 5 months after a 60 mg shot. It did not build up with repeated doses every 6 months. The current label gives a peak level but no time to peak.
