Short answer: Both are forms of carnitine, which the body also makes.1 L-carnitine is mostly studied for weight: pooled trials show small drops in weight and waist size.2 Acetyl-L-carnitine (ALCAR) is studied for memory and mood; a Cochrane review found no clear benefit in Alzheimer disease, while a 2026 review found less depression.34 No trial compared the two.
| L-carnitine | Acetyl-L-carnitine (ALCAR) | |
|---|---|---|
| What it is | The main form of carnitine, made in the liver, kidneys and brain and found in red meat1 | Another carnitine compound; its absorption has not been well studied1 |
| Main research use | Weight: pooled trials of 150 to 4,000 mg a day found small drops in weight, BMI and waist size2 | Memory and mood34 |
| Memory | Not its main research area in the sources checked | Small effect in a 2003 meta-analysis of mild cognitive impairment and early Alzheimer disease; Cochrane review found no clear benefit on thinking53 |
| Mood | Not its main research area in the sources checked | Less depression than placebo across 14 trials4 |
| Side effects | About 3 g a day can cause nausea, cramps, diarrhea and a fishy body odor1 | Well tolerated in memory trials5 |
Carnitine plays a key role in making energy. Healthy people make enough of it, and red meat is the richest food source.1 An umbrella analysis of 8 meta-analyses found L-carnitine lowered weight, BMI and waist size by small amounts over 8 to 30 weeks, with high variation between trials.2 NIH calls the evidence for athletic performance mixed. Gut bacteria turn unabsorbed carnitine into TMAO, which may raise heart disease risk; NIH says this is not well understood.1
A 2003 meta-analysis of trials lasting 3 to 12 months, at 1.5 to 3 g a day, found a small advantage for ALCAR over placebo in mild cognitive impairment and early Alzheimer disease.5 A Cochrane review of 11 Alzheimer trials found better global ratings at 12 and 24 weeks but no evidence of benefit on thinking, and concluded ALCAR is unlikely to be an important treatment.3
For mood, a 2026 review of 15 studies with 809 people found ALCAR reduced depressive symptoms compared with placebo, with fewer side effects than standard antidepressants.4
They have been studied for different things, and no trial has compared them. NIH notes carnitine can interact with some medicines and may cause seizures in people with seizure disorders.1 This is general information, not personal advice.
