Short answer: The Depo-Provera CI label gives one storage line: vials must be stored upright at controlled room temperature, 68-77°F (20-25°C). It gives no storage text for the prefilled syringe and no fridge, freezing or light instruction. Both the vial and the syringe are shaken vigorously just before use. The shot is given by a healthcare professional. Source: Depo-Provera CI label, sections 2.1 and 16.
Section 16: “Vials MUST be stored upright at controlled room temperature 20° to 25°C (68° to 77°F) [see USP].”

Depo-Provera CI is a suspension, not a solution. Section 2.1: both the vial and the prefilled syringe “should be vigorously shaken just before use to ensure that the dose being administered represents a uniform suspension.” The syringe label says the same: shake vigorously before use.
The date. The dose is 150 mg every 3 months (13 weeks). If more than 13 weeks pass between shots, the label says the physician should determine that the patient is not pregnant before giving the next one. See a late Depo-Provera shot, the Depo-Provera dosing schedule and Depo-Provera side effects.
