Short answer: The Yorvipath label starting dose is 18 mcg injected under the skin once a day. The dose is changed in 3 mcg steps based on blood calcium, no more than every 7 days up or every 3 days down, within 6 to 30 mcg a day. Each daily dose is one injection. Active vitamin D and calcium supplements are cut back on the starting day, and calcium is checked 7 to 10 days after every change.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Yorvipath is approved for: Yorvipath is a parathyroid hormone analog (PTH(1-34)) approved to treat hypoparathyroidism (low parathyroid hormone) in adults. It was not studied for sudden low parathyroid hormone right after surgery, and its dose-adjustment plan was only tested in adults whose albumin-corrected blood calcium was first brought to at least 7.8 mg/dL with calcium and active vitamin D (section 1).

Yorvipath is injected under the skin of the abdomen or front of the thigh once a day, rotating the site daily. The first doses are given where the person can sit or lie down (section 2.6).
Only one injection is used to reach the daily dose; two injections make the dose less predictable (sections 2.1 and 5.2). Each pen covers 3 dose settings, so a dose change can mean a different pen (section 3).
Pens are refrigerated until first use, then kept at room temperature below 30 C (86 F) and thrown away 14 days after first use (section 16.2).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Item | What the label says |
|---|---|
| Before the first dose | Within two weeks before the first dose, confirm serum 25(OH) vitamin D is within the normal range and albumin-corrected serum calcium is ≥ 7.8 mg/dL |
| Starting dosage | 18 mcg once daily |
| Dose steps | 3 mcg increments or decrements |
| How often | Do not increase more often than every 7 days. Do not decrease more often than every 3 days |
| Dosage range | 6 to 30 mcg once daily |
| Maximum | 30 mcg subcutaneously once daily |
| Calcium checks | Within 7 to 10 days after the first dose and any dose change; at a minimum every 4 to 6 weeks once the maintenance dosage is achieved |
| Calcium 12 mg/dL or greater | Withhold for 2 to 3 days and then recheck serum calcium |
| Albumin-Corrected Serum Calcium | Current Active Vitamin D (calcitriol) Intake | Adjust Active Vitamin D (calcitriol) Intake | Adjust Calcium Supplements |
|---|---|---|---|
| ≥8.3 mg/dL | >1 mcg/day | Reduce calcitriol dosage by ≥50% | Maintain current calcium dosage |
| ≥8.3 mg/dL | ≤1 mcg/day | Discontinue calcitriol | Maintain current calcium dosage |
| ≥7.8 to <8.3 mg/dL | Any amount | Reduce calcitriol dosage by ≥50% | Maintain current calcium dosage |
| ≥7.8 mg/dL | Not currently on active vitamin D | Not applicable | Reduce calcium daily dosage by at least 1500 mg or discontinue if current calcium daily dosage is ≤1500 mg/day |
Applied on the day Yorvipath is started or the dose is raised. Where calcium is stopped, the label says dietary calcium supplements of 600 mg/day or less of elemental calcium may be continued instead.

The maintenance dose is the one that keeps blood calcium in the normal range without active vitamin D or therapeutic calcium doses; calcium to meet daily dietary needs may continue. Figure 1 sets the next step for Yorvipath, active vitamin D and calcium by calcium range: below 8.3, 8.3 to 10.6, and 10.7 to 11.9 mg/dL.
If calcium stays low at 30 mcg, the label says adding or restarting calcium or active vitamin D, or other treatment, may be considered. Some people need a higher dose over time to keep the same effect.
If treatment is delayed or interrupted for 3 days or more, the person is checked for low calcium and blood calcium may be measured. Calcium and active vitamin D may be restarted or raised. Yorvipath is restarted at the previous dose as soon as possible, and calcium is checked 7 to 10 days later.
Each pack holds 2 prefilled pens and 30 needles: 28 injections plus 2 spare needles.
The label says: "Take YORVIPATH as soon as possible if a dose is missed by less than 12 hours. Skip the missed dose if the dose has been missed by more than 12 hours. Take the next dose as scheduled." The Medication Guide says the same.
More on this: missed a dose of Yorvipath.
In Dose Track you set up Yorvipath as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
