Short answer: The NovoLog Mix 70/30 label gives each dose under the skin within 15 minutes before starting a meal, typically twice daily, with each dose meant to cover 2 meals or a meal and a snack. In type 2 diabetes it may also be given after the meal starts. The dose is individualized, with no fixed starting dose. The label has no missed-dose rule.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What NovoLog Mix 70/30 is approved for: NovoLog Mix 70/30 is a premixed insulin approved to improve blood sugar control in adults with diabetes. It is not recommended for diabetic ketoacidosis, and its fixed proportions do not allow separate basal and mealtime dose changes (section 1).

NovoLog Mix 70/30 is a suspension mixed right before use: a vial is rolled gently 10 times, and a FlexPen is rolled 10 times and then turned upside down 10 times, until evenly white and cloudy. It is injected right away under the skin of the abdomen, buttocks, thigh or upper arm, rotating sites (section 2.1).
It is U-100 (100 units/mL), 70% insulin aspart protamine and 30% insulin aspart, in a 10 mL multiple-dose vial and a 3 mL FlexPen that dials in 1-unit steps. It is not given into a vein, used in a pump or mixed with other insulins (sections 2.1 and 3).

NovoLog Mix 70/30 is not recommended for treating diabetic ketoacidosis, and its fixed proportions do not allow separate basal and mealtime dose adjustments.
Dose changes may be needed with changes in physical activity, meal content or timing, kidney or liver function, or during acute illness. Glucose is checked more often during any change to the insulin regimen.
It is not used if it looks clear or has solid particles. The FlexPen is used with caution by people with poor vision who rely on the clicks.
Unused NovoLog Mix 70/30 is stored in the refrigerator, never frozen, and kept from excess heat or light. The needle is removed after each FlexPen injection.
Patients are taught how to handle illness, stress, an inadequate or skipped insulin dose, an accidental extra dose, too little food and skipped meals. The label does not spell out what to do in each case.
The NovoLog Mix 70/30 label gives no missed-dose rule. The Patient Information says not to inject it when not planning to eat within 15 minutes (type 1 diabetes), but neither it nor the prescribing information says what to do after a missed dose.
When switching from another insulin to NovoLog Mix 70/30, a different dose may be needed.
Any change of insulin strength, maker, type, site or method is made under close medical supervision with more frequent glucose checks. In type 2 diabetes, other diabetes medicines may need adjusting.
In Dose Track you set up NovoLog Mix 70/30 as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
