Short answer: The Humulin N label gives no fixed starting dose or number of daily injections: the dose is individualized by metabolic needs, glucose monitoring and glucose goals. It is injected only under the skin, never into a vein or by insulin pump. It may be mixed in the syringe with Humulin R or Humalog, drawing those in first. The label has no missed-dose rule.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Humulin N is approved for: Humulin N is an intermediate-acting human insulin approved to improve blood sugar control in adults and children with diabetes (section 1).

Humulin N is injected under the skin of the abdomen, thigh, upper arm or buttocks, rotating sites within the same region. It is not given into a vein or used in an insulin pump. It should look evenly cloudy after mixing and is not used if particles are seen (sections 2.1 and 2.2).
It is U-100 (100 units/mL) in a 10 mL multiple-dose vial and a 3 mL KwikPen that dials in 1-unit steps. It may be used with a mealtime insulin (sections 2.2, 2.4 and 3).
The table below is the label's schedule, from the Dosage and Administration section. Numbers are copied exactly; the section is in brackets.
| Situation | What the label says |
|---|---|
| Dose | Individualized by metabolic needs, blood glucose monitoring and glucose goal; no fixed starting dose is given |
| Route | Under the skin only; not into a vein and not by insulin pump |
| Mixing with Humulin R | Draw Humulin R into the syringe first; inject immediately after mixing |
| Mixing with Humalog | Draw Humalog into the syringe first; inject immediately after mixing |
| Changing from another insulin | The dose may need adjusting |
| Presentation | Unopened, room temp up to 86 F | Unopened, refrigerated 36-46 F | In use, room temp up to 86 F | In use, refrigerated |
|---|---|---|---|---|
| 10 mL multiple-dose vial | 31 days | Until expiration date | 31 days | 31 days |
| 3 mL KwikPen | 14 days | Until expiration date | 14 days | Do not refrigerate |
At room temperature, a vial can be used for 31 days in total and a KwikPen for 14 days in total, counting both unopened and opened time.
Dose changes may be needed with changes in physical activity, meals, kidney or liver function, or during acute illness. Glucose is checked more often during any change to the insulin regimen.
The dose may need adjusting when Humulin N is used with certain other drugs.
The label advises caution with the KwikPen for people with poor vision who rely on its clicks.
Humulin N is protected from heat and light, never frozen, and not used if it has been frozen.
Patients are taught how to handle illness, stress, a skipped or inadequate insulin dose, an accidental extra dose, too little food and skipped meals. The label does not spell out what to do in each case.
The Humulin N label gives no missed-dose rule. Section 17 says patients are instructed on handling an inadequate or skipped insulin dose, but neither the prescribing information nor the Patient Information says what to do.
Dose adjustment may be needed when changing from another insulin to Humulin N.
Any change of insulin strength, maker, type, site or method is made under close medical supervision with more frequent glucose checks.
In Dose Track you set up Humulin N as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
