Testosterone Pellets: pellet dosing, label use and warnings

Pellet strength · Approved vs off-label use · Half-life notes

What is testosterone pellets?

Testopel (Endo) is crystalline testosterone pressed into small pellets that are implanted under the skin, where they release testosterone slowly for months. It is used for men with low testosterone from testicular or pituitary causes, and for selected boys with clearly delayed puberty.

Also known as: Testopel, testosterone implant.

Pellet strength

Each Testopel pellet holds 75 mg of testosterone and is about 3.2 mm across and 9 mm long. Pellets come one per vial, in boxes of 10 or 100.

Approval status and how it is used

The Testopel label approves it as testosterone replacement in men with low or absent testosterone from primary hypogonadism (testicular failure) or hypogonadotropic hypogonadism, and to start puberty in carefully selected boys with clearly delayed puberty. The label says safety and effectiveness in age-related ("late-onset") hypogonadism have not been established. Its dose guideline is 150 to 450 mg (two to six pellets) implanted under the skin every 3 to 6 months, sized at two 75 mg pellets for each 25 mg of testosterone propionate a week by injection; the effect usually lasts 3 to 4 months, sometimes 6. The diagnosis is first confirmed by two low morning testosterone results.

The label has no boxed warning. It is not used with breast cancer or known or suspected prostate cancer, and it can harm a female fetus. Warnings cover infection and pellets pushing out at the implant site (mostly in the first month), blood clots, heart risk and blood pressure, liver and prostate effects, and abuse. Because pellets release over months, the dose is hard to adjust once implanted. The label lists side effects without percentages. Full details are on the Testopel side effects and Testopel dosing schedule pages, and Testopel vs testosterone cypionate compares pellets with shots.

Testosterone pellets half-life and time to peak

  • Half-lifeNot listed
  • Time to peak (Tmax)Not listed

The Testopel label does not give a half-life or time to peak. It says the pellets slowly release testosterone for a long-acting effect that usually lasts three to four months and sometimes up to six, so we do not list a number.

How to track testosterone pellets in Dose Track

  1. Add testosterone pellets as a custom compound. Start typing its name and Dose Track fills in the details from its research library for you to check, and it sits next to everything else you take.
  2. Log each pellet insertion with the number of pellets, so your history shows when the next one is due and lines up with your testosterone results.
  3. Set reminders on your own schedule, whether that is daily, weekly or every few days.
  4. Your log is stored on your phone and no account is needed. Data only leaves it for features you choose, like AI answers and lab reading.
