Short answer: Flibanserin has a half-life of about 11 hours. Levels peak about 0.8 hours (fasted) after a dose. People vary, so treat these as estimates.
Flibanserin acts on serotonin receptors in the brain (it activates 5-HT1A and blocks 5-HT2A). It is sold as Addyi (Sprout Pharmaceuticals), a tablet taken once a day at bedtime for low sexual desire in women.
Also known as: Addyi.
Addyi comes as a 100 mg tablet, taken once a day at bedtime.
The Addyi label approves flibanserin for women under 65 with acquired, generalized hypoactive sexual desire disorder (HSDD): low desire that causes marked distress, began after a time without it, and is not caused by another condition, relationship problems or a drug. It is not for men or for boosting sexual performance. Label dosing is 100 mg once a day at bedtime only, because daytime dosing raises the risk of low blood pressure, fainting and sleepiness. It is stopped after 8 weeks if there is no improvement.
Addyi has a boxed warning titled "Hypotension and syncope in certain settings": after one or two drinks, the bedtime dose waits at least 2 hours; after three or more, that night's dose is skipped. Moderate or strong CYP3A4 inhibitors (such as fluconazole, clarithromycin or grapefruit juice) and any liver impairment are contraindicated. In trials, dizziness (11.4% vs 2.2%), sleepiness (11.2% vs 2.9%) and nausea (10.4% vs 3.9%) were the most common side effects. Full numbers are on the Addyi side effects, Addyi dosing schedule and missed Addyi dose pages, and Addyi vs Vyleesi compares it with bremelanotide.
The Addyi label gives a mean terminal half-life of about 11 hours (13.5 hours in people who break it down slowly through CYP2C19). The median time to peak was 0.8 hours fasted, and food pushed it to 0.9 to 1.8 hours.
