Drospirenone/Estradiol (Angeliq): label uses, potassium warning and half-life

Tablet strengths · Approval status · Half-life drospirenone about 39 hours (0.5 mg/1 mg strength)

Short answer: Drospirenone/Estradiol has a half-life of drospirenone about 39 hours (0.5 mg/1 mg strength). Levels peak drospirenone about 1 hour; estradiol about 2 hours after a dose. People vary, so treat these as estimates.

What is Drospirenone/Estradiol?

Angeliq (Bayer) combines estradiol, the main estrogen, with drospirenone, a progestin related to spironolactone, for women with a uterus after menopause. Like spironolactone, drospirenone blocks aldosterone, which can raise potassium.

Also known as: Angeliq, drospirenone.

Tablet strengths

Angeliq comes as 0.25 mg drospirenone/0.5 mg estradiol and 0.5 mg drospirenone/1 mg estradiol tablets, 28 to a pack, taken every day with no break.

Approval status and how it is used

The Angeliq label approves both strengths for moderate to severe hot flashes of menopause, and the 0.5 mg/1 mg strength also for moderate to severe vaginal atrophy symptoms. Label dosing is one tablet a day, swallowed whole, with or without food, ideally at the same time; packs follow each other with no break. A tablet more than 24 hours late is not taken.

Its boxed warning is titled "Cardiovascular disorders, probable dementia, breast cancer, and endometrial cancer", based on the Women's Health Initiative. In November 2025 FDA said it is starting to remove the boxed warnings about heart disease, breast cancer and probable dementia from menopause hormone products, but not the endometrial cancer warning for systemic estrogen-alone products. The label version we read still carries the full boxed warning. It is not used with kidney impairment, liver disease or adrenal insufficiency, and the label urges care with other drugs that raise potassium, such as NSAIDs, potassium-sparing diuretics, ACE inhibitors and ARBs. Breast pain was 17.9% on the 1 mg estradiol strength. Full numbers are on the Angeliq side effects, Angeliq dosing schedule and missed Angeliq dose pages. Storage: Angeliq.

Drospirenone/Estradiol half-life and time to peak

  • Half-lifedrospirenone about 39 hours (0.5 mg/1 mg strength)
  • Time to peak (Tmax)drospirenone about 1 hour; estradiol about 2 hours

The Angeliq label says drospirenone peaks about 1 hour after a tablet, with 76 to 85% absorbed, and estradiol a median of about 2 hours after (range 0.3 to 10 hours). At steady state on the 0.5 mg/1 mg strength, the drospirenone half-life was 39.2 hours. The label table gives no estradiol half-life.
