Short answer: Conjugated Estrogens/Bazedoxifene has a half-life of about 17 hours (estrone) and 30 hours (bazedoxifene). Levels peak about 6.5 hours (estrone) and 2.5 hours (bazedoxifene) after a dose. People vary, so treat these as estimates.
Duavee (Pfizer) combines conjugated estrogens with bazedoxifene, an estrogen agonist/antagonist. The bazedoxifene part protects the uterine lining from the estrogen, so women with a uterus do not need a progestin with it. It is taken once a day for hot flashes and to prevent osteoporosis after menopause.
Also known as: Duavee, bazedoxifene.
Duavee comes in one strength: conjugated estrogens 0.45 mg with bazedoxifene 20 mg. Tablets stay in the blister in a foil pouch; once the pouch is opened, the pack is used within 60 days.
The Duavee label approves it for women with a uterus to treat moderate to severe hot flashes of menopause and to prevent osteoporosis after menopause. Label dosing is one tablet a day, swallowed whole, with or without food. Women taking it do not take a progestin, extra estrogen or another estrogen agonist/antagonist.
Its boxed warning is titled "Endometrial cancer, cardiovascular disorders, and probable dementia", based on the estrogen-alone Women's Health Initiative findings on stroke, blood clots and probable dementia. In November 2025 FDA said it is starting to remove the boxed warnings about heart disease, breast cancer and probable dementia from menopause hormone products, but not the endometrial cancer warning for systemic estrogen-alone products. It is not used with unexplained uterine bleeding, breast cancer, estrogen-dependent cancer, a current or past blood clot, stroke or heart attack, liver disease, clotting disorders or pregnancy. In four placebo trials, muscle spasms (9% vs 6%), nausea (8% vs 5%) and diarrhea (8% vs 5%) were the most common side effects. Full numbers are on the Duavee side effects, Duavee dosing schedule and missed Duavee dose pages, and Duavee vs Prempro compares it with estrogen plus progestin.
The Duavee label gives steady-state peak times of 6.5 hours for baseline-adjusted total estrone (standing in for the conjugated estrogens) and 2.5 hours for bazedoxifene. After a single dose, estrone was removed with a half-life of about 17 hours and bazedoxifene about 30 hours; with severe liver impairment the bazedoxifene half-life rose from 32 to 50 hours.
